Help Ilse naar een toekomst met perspectief

Met deze crowdfunding hoop ik dat we de behandeling voor mijn lieve vriendin Ilse kunnen bekostigen die haar helpt te herstellen van haar klachten als gevolg van traumatisch hersenletsel. Zodat Ilse weer een ‘normaal’ leven kan leiden. Waarbij ze weer kan deelnemen aan de maatschappij, plezier kan hebben en zich gezonder en daarmee gewoon goed voelt.

Het verhaal van Ilse

Het was oktober 2019 en Ilse was 25. Een vrolijke en hardwerkende meid die volop van het leven genoot, actief was en alle zin had om iets van haar leven te maken. Ze stond ‘s ochtends op het punt de deur uit te gaan naar haar stageplek. Totdat ze uitgleed in haar kamer.

Ze viel en maakte een harde klap. Ze was kort buiten bewustzijn, verschrikkelijk duizelig en kon alleen maar overgeven. Een scan in het ziekenhuis liet gelukkig geen interne bloedingen zien en het advies was: “Je hebt een hersenschudding, rust uit en je wordt vanzelf weer de oude."

Helaas verliep het anders. Want die verbetering bleef uit. Maanden later ging het alleen maar slechter. Ilse bleek een whiplash en Postcommotioneel Syndroom (PCS) te hebben. Dit was en is nog steeds een enorme teleurstelling. En ondanks cognitieve verbetering door ergotherapie in 2020, staat haar en daarmee ook ons leven op dit moment, ruim 4 jaar later, nog volledig in het teken van het ongeluk. In haar doen en laten wordt Ilse dagelijks erg beperkt doordat bijvoorbeeld geluiden, bewegingen, trillingen en licht zorgen voor vreselijke hoofdpijn en een onwel gevoel. Ze is duizelig bij liggen en draaiingen van haar hoofd en vanuit haar nek heeft ze veel plaatselijke en uitstralende zenuwpijn. Reizen met het OV lukt niet meer, een kopje thee met vriendinnen kan alleen thuis, sporten en dansen doet ze allang niet meer - bijna alles wat vroeger normaal was en wat ze leuk vond, is nu niet haalbaar. Het is een onzeker en eenzaam bestaan. En daarmee wanhopigmakend en uitzichtloos ondanks de pijnlijke behandelingen tot nu toe.

Het breekt mijn hart dat Ilse al zolang in onzekerheid en met haar pijn moet leven. Activiteiten die Ilse’s leven ooit kleur gaven, lukken nu al 4 jaar niet. Wat verder meespeelt is dat haar letsel niet aan de buitenkant te zien is. Mensen zien Ilse wanneer ze ‘redelijk goed’ is, maar helaas geeft dat een vertekend beeld.

Ilse en ik zijn samen op zoek naar hoop. Hoop dat het beter gaat worden. Voor haar persoonlijk maar letterlijk ook dat wij ons leven samen weer een andere inhoud kunnen gaan geven.

Over deze medische crowdfunding en ons plan

Ik heb deze crowdfunding opgezet om Ilse's behandeling in Amerika te kunnen bekostigen. Caring Medical is een gespecialiseerd nekcentrum in Florida dat twee dingen aanbiedt die in Nederland en Europa helaas (nog) niet voorhanden zijn: de diagnostiek van Ilse’s hoogcervicale nekinstabiliteit én de behandeling daarvan.

De benodigde scans en specifieke behandeling die zij nodig heeft zijn relatief nieuw en onbekend in Nederland. Hierdoor wordt de behandeling niet vergoed door zorgverzekeraars. De resultaten en verhalen van lotgenoten zijn dusdanig veelbelovend dat wij zeer binnenkort voor deze behandeling willen gaan. Omdat Ilse’s conditie verslechtert, voelt deze optie voor ons als nu of nooit. Gelukkig kunnen we, als het ons lukt, al in januari 2024 terecht.

Jarenlange zoektocht naar een oplossing

Ilse stond in 2020 op het punt te starten met een revalidatietraject, maar vanwege COVID-19 sloten de deuren zich vlak voor haar opname. In de corona-jaren daarna kreeg ze wekelijks fysiotherapie en ergotherapie en stopte ze tijdelijk met haar studie, maar veel fysieke en neurologische klachten bleven. Gemaakte scans lieten ook toen geen aantoonbaar letsel zien en het devies was: “Sorry, maar we kunnen op dit vlak weinig voor je betekenen of verbeteren.”

Ilse kreeg er steeds meer nieuwe klachten bij. Nadat we in 2022 informatie over de aandoening Occipitalis Neuralgie vonden, kwam Ilse in aanmerking voor behandelingen in een pijnkliniek. Hier werd ze behandeld met medicijnen en behandeling van de zenuwen met corticosteroïden. De behandelingen hebben een half jaar geduurd. Ondanks de grote hoop die we in die periode voorzichtig voelden, kwam de pijn vanuit de nek steeds weer terug. De behandeling was te veel gericht op pijnbestrijding en niet gericht op het aanpakken van de oorzaak van de klachten, bleek achteraf. We voelden ons wanhopig en zaten er compleet doorheen.

Prolotherapie: hoop voor Ilse?

Al onze energie ging op aan het steeds maar verder zoeken naar oplossingen: wie en wat kan ons helpen?

Begin 2023 kwamen we in aanraking met prolotherapie. Een behandeltraject met injectie-therapie gericht op het verstevigen van de elastische banden om spieren en wervels heen. Het blijkt dat letsel aan die banden (ligamenten) kan zorgen voor instabiele wervels en dat die op hun beurt allerlei neurologische- en pijnklachten in stand kunnen houden. We hadden er nog nooit van gehoord, maar Ilse voelde zich zó (h)erkend in de beschreven symptomen en verhalen van lotgenoten. Ze was niet alleen! Patiënten die op traditionele MRI-scans geen afwijkingen laten zien, maar waar met geavanceerde röntgentechnieken pas bij het bewegen van hoofd en nek zichtbaar wordt wat er van binnen niet goed gaat.

Ilse kreeg sinds april 2023 meerdere prolotherapie-behandelingen in haar rug en nek. Na enkele weken merkte ze versteviging: ze kon haar hoofd weer beter ‘dragen’. Kort daarna had ze zelfs voor het eerst in drie jaar tijd geen diepe pijn in haar nek. We konden wel een gat in de lucht springen. Eindelijk iets dat werkte!

Ondanks de diverse behandelrondes in Nederland en België tot nu toe, blijft Ilse hoog in de nek intense pijn houden. De behandelende artsen geven aan dat het gebied waar Ilse’s problematiek hoogstwaarschijnlijk vandaan komt, de hoogcervicale nekwervels, vanwege de complexiteit niet bij hen (maar ook niet in Europa behandeld) kan worden. Hoe graag deze artsen ook willen, de expertise en middelen hebben ook zij nog niet.

Over de behandeling bij Caring Medical

Bij Caring Medical in Florida bieden ze een uniek behandeltraject voor langdurig herstel. In plaats van statische beelden maken zij röntgen video’s. Hiermee kunnen ze, bij beweging van Ilse’s nek, zien hoe de hogere nekwervels ten opzichte van elkaar bewegen. Ook voeren ze allerlei andere tests uit om erachter te komen waar het precies misgaat in 'de keten'. Want vanuit haar nek ontstonden ook kaak- en oorproblemen. Vervolgens behandelen ze dat specifieke gebied onder begeleiding van diagnostische tooling. Deze combinatie is simpelweg (en jammer genoeg) ergens anders niet aanwezig.

Behandelduur zo’n 6 tot 12 weken

Caring Medical geeft aan dat we rekening moeten houden met zo’n 6 tot 12 weken behandeltijd. Op basis van in-house scans en testen wanneer we daar zijn, zal duidelijk worden hoe lang het precies wordt en wat nodig is. Ilse zal elke 1,5 tot 2 weken een behandeling ondergaan, om daar vervolgens tussentijds van bij te komen. Gedurende en na de behandelingen worden opnieuw scans gemaakt om de effecten in kaart te brengen.

Dit betekent ook dat we die periode in Florida moeten verblijven. Ik zal grotendeels remote gaan werken en we gaan op zoek naar een passend onderkomen dat dit mogelijk maakt.

Toelichting kosten

Behandeling: €18.500,-

Vluchten: €8.000,-

Verblijf en vervoer: €3.500,-

De definitieve behandeling wordt pas duidelijk in Florida en daarom zijn de kosten hierboven een globale inschatting op basis van de informatie die Caring Medical ons gaf. De vlucht naar de Verenigde Staten zal vanwege Ilse’s huidige beperkte gezondheid zo rustig mogelijk moeten verlopen. Om die reden willen we vliegen met KLM (Business Class) en in overleg met de vliegmaatschappij en het vliegtuigpersoneel zorg dragen voor een zo soepel mogelijke vliegreis voor Ilse.

De verwachtte kosten voor het gehele traject zijn hoog en worden niet vergoed. Toch moeten we dit gaan doen, voordat Ilse’s kans op verbetering helemaal is uitgesloten. Wij willen ervoor gaan en doen bij dezen een beroep op jullie steun. Mocht de behandeling aanslaan en voor verbetering zorgen, dan wil Ilse in de nabije toekomst heel graag anderen in soortgelijke situaties helpen.

Help Ilse towards a future with perspective

With this crowdfunding for my girlfriend Ilse, I hope that we can finance the necessary treatment that will hopefully help her recover from her pain as a result of traumatic brain injury. So that Ilse can lead a 'normal' life again. Where she can participate in society, have fun and feel healthier and therefore simply good.

Ilse's story

It was October 2019 and Ilse was 25. A cheerful and hardworking girl who enjoyed life, was active and had every desire to make something of her life. She was about to leave the house for her internship in the morning. Until she slipped in her room.

She fell and hit her head hard. She was briefly unconscious, terribly dizzy and could only vomit. Fortunately, a scan in the hospital showed no internal bleeding and the advice was: "You have a concussion, just rest and you will get back to normal."

Unfortunately, things turned out differently. Because improvements did not come. Months later things only got worse. Ilse turned out to have a whiplash and Post-concussion Syndrome (PCS). This was a huge disappointment. And despite cognitive improvement through occupational therapy in 2020, her and therefore our lives today are still completely dominated by the accident. Ilse is very limited in her actions because, for example, noise, vibrations and light cause immediate headaches and an unwell feeling. She feels dizzy when she turns her head and has a lot of local and radiating pain from the neck. Traveling by public transport is no longer possible, a cup of tea with friends can only be done at home, she no longer does sports and dancing - almost everything that used to be normal and that she enjoyed, is now not doable. It is an uncertain and lonely existence. And therefore desperate and hopeless, despite the painful treatments so far.

It breaks my heart that Ilse has had to live in uncertainty and with her pain for so long. Activities that once gave Ilse's life color, have not been possible for 4 years now. Next to that, her injury is not visible from the outside. People see Ilse when she is 'pretty good', but unfortunately that gives a distorted picture.

Ilse and I are longing for hope. Hope that it will get better. For her personally, but also so that we can start looking towards the future again.

About this medical crowdfunding and our plan

I set up this crowdfunding to help finance her treatment in America. Caring Medical is a specialized neck center in Florida that offers two things that are unfortunately not (yet) available in the Netherlands and Europe: the diagnosis of Ilse's upper cervical neck instability and its treatment.

The required scans and specific treatment she needs are relatively new and unknown in the Netherlands. As a result, the treatment is not reimbursed by health insurers. The results and stories from patients at Caring Medical are so promising. Because Ilse's condition is worsening, this option feels like now or never to us. Fortunately, if we succeed, we could start treatment in January 2024 already.

Years of search for a solution

Ilse was about to start a rehabilitation program in 2020, but due to COVID-19 the doors closed just before her admission. In the subsequent years, she received weekly physiotherapy and occupational therapy and temporarily stopped her studies, but many physical and neurological complaints remained. Scans showed no demonstrable injury and the advice was: “Sorry, but there is little we can do or improve for you in this area.”

Ilse started getting more and more pain and neurological symptoms. After we found information about the condition Occipitalis Neuralgia in 2022, Ilse became eligible for treatment in a pain clinic. Here she was treated with medications and nerve treatment with corticosteroids. The treatments lasted six months. Despite the great hope that we cautiously felt during that period, the pain from the neck kept coming back. The treatment was focused too much on pain relief and not on tackling the cause of the symptoms, it turned out afterwards. We felt desperate and completely exhausted.

Prolotherapy: hope for Ilse?

All of our energy was spent on searching for options: who and what can help us?

At the beginning of 2023, we learned about prolotherapy. A treatment program with injection therapy aimed at strengthening the elastic ligaments around muscles and vertebrae. It appears that injured ligaments can cause unstable vertebrae and that these in turn can maintain all kinds of neurological and pain complaints. We had never heard of it, but Ilse recognized herself so much in the symptoms and stories of fellow patients. She was not alone! Patients who showed no abnormalities on traditional MRI scans, but do so with advanced X-ray techniques when the head and neck move.

Ilse has received several prolotherapy treatments in her back and neck in the Netherlands since April 2023. After a few weeks she noticed that she could 'bear' her head better again. Shortly afterwards, for the first time in three years, she had no deep pain in her neck. We were so excited. Finally something that worked!

Despite the various rounds of treatment in the Netherlands and Belgium so far, Ilse continues to have intense pain high in the neck. The doctors indicate that the area from which Ilse's problems most likely originate, the upper cervical vertebrae, cannot be treated by them (but also not in Europe) due to its complexity. As much as these doctors would like to, they do not yet have this expertise or the treatment options.

About the treatment at Caring Medical

Caring Medical in Florida offers a unique treatment program for long-term recovery. Instead of static images, they make X-ray videos. This allows them to see how the higher cervical vertebrae move relative to each other when Ilse's neck moves. They also carry out all kinds of other tests to find out where exactly things go wrong in related areas. They then treat that specific area under the guidance of diagnostic tooling. This combination and expertise is simply (and unfortunately) not present elsewhere.

Treatment duration about 6 to 12 weeks

Caring Medical indicates that we should expect approximately 6 to 12 weeks of treatment time. Based on in-house scans and tests when we arrive there, it will become clear exactly how long it will take and what is needed. Ilse will undergo treatment every 1.5 to 2 weeks and then recover in between. During and after the treatments, scans are made again to map the effects.

This also means that we have to stay in Florida for that period. I will work remotely for the most part and we will look for suitable accommodation that makes this possible.

Treatment: €18,500

Flights: €8,000

Accommodation and transport: €3,500

The definitive treatment plan will become clear once in Florida and therefore the costs above are a global estimate based on the information Caring Medical provided. Due to Ilse's current limited health, the flight to the United States has to be as calm as possible. For that reason, we want to fly with KLM (Business Class) and, in consultation with the airline and the flight crew, ensure the smoothest possible flight for Ilse.

Will you help?

The costs for the entire process are high and are not reimbursed. Yet we have to do this before Ilse's chance of improvement is completely excluded. We want to go for it and hereby appeal to your support in our journey. If the treatment works and leads to improvement, Ilse wants to help others in similar situations in the near future.
