UPDATE Please help us figure out the sudden passing

Please help us figure out why our father suddenly passed. He was admitted to the hospital and received a full course of IV antibiotics for his pneumonia and the doctor said the pneumonia resolved and he was ready for discharge to a Nursing rehab for physical therapy. On 7/26/2024 at 8:47am a Discharge Case Manager from the hospital called us and left a message stating my father was accepted to a Nursing facility and wanted to go over the discharge paperwork .

I immediately took the day off and was getting ready to head over to the hospital. My Aunt , who is Spanish speaking got to the hospital before me. So she wasn’t able to tell me anything that was said while she was there. As im hearing over, i receive another call from a Palliative Nurse explaining that I needed to gather my family to come see my father asap at the hospital because things had changed. And that very day he passed.

I am trying to figure out what and why this happened and i want the hospital to be held responsible for whatever went wrong, but we need a private autopsy, toxicology labs, medical records to start. Not to mention all the other medical and death related expenses to cover. Please help us get some closure, justice, and hold someone accountable.
