Dear Family and Friends,
 
I would like to introduce you to Hashim Raza. He is residing in a small town in Pakistan.
 
Since last February, he has been fighting cancer (you can refer to the pinned post on his profile for further details).
 
 
The doctors diagnosed him with STAGE 4 STOMACH and INTESTINAL CANCER.
Hashim is only 24 years old; he needs our Duas and Financial support to fight in this critical phase of his life.

Timeline of his fight:
  • March 16, 2021: esophagogastroduodenoscopy : pyloric stenosis
  • March 22, 2021: stomach biopsy: Mild chronic gastritis (No H. Pylori activity intestinal metaplasia or dysplasia seen)
  • March 18, 2021: CT ABD PELVIS with I/V & Oral contrast. Mild circumferential mural thickening at pylorus up to 10mm with mild stenosis pyloric canal resulting in distension of the stomach. Visualized liver, lower chest skeleton normal. No locoregional lymphadenopathy
  • April 30, 2021: Re-Endoscopy: narrowed pylorus seen with resistance passing through it. Balloon dilatation of pylorus up to 12mm done.
  • May 5, 2021:
  • EUS: Diffusely infiltrated all 4 layers of the pyloric end of the stomach, causing luminal compromise. Multiple perigastric lymph nodes have seen less than 1cm.MALToma with GOO?
  • Stomach biopsy: fibrinous material with a single fragment of smooth muscle, consistent with sampling from gastric wall tissue. No evidence of malignancy.
  • FNA Perigastric lymph node: blood only, suboptimal for diagnosis.
  • May 6, 2021: laparoscopic gastrojejunostomy rouxen y gastric bypass surgery done.. no biopsy taken.
  • The patient remained asymptomatic for two months.
  • Sep 9, 2021: CT ABDOMEN PELVIS with contrast: circumferential thickening of 23mm pyloric region of the stomach with maximal wall thickness up to 19mm with the distended proximal stomach.
  • Sep 29, 2021: endoscopy: gastrojejunostomy site edematous and stenosed with overcrowding of mucosal folds. Repeat endoscopy and balloon dilatation were done four times from September 29, 2021, to April 11, 2021; no-repeat biopsy was done.
  • Dec 15, 2021: CT CHEST ABD PELVIS: Small Bowel loops clumped indicate abdominal cocoon formation with diffuse peritoneal thickening and possible diffuse peritoneal sclerosis. Mild large bowel edema. Moderate abdominopelvic ascites. Lungs and bones are unremarkable.
  • Dec 30, 2021: Peritoneal (ascitic ) fluid cytology: suspicious for malignant cells CK 7 +ve, CEA scattered atypical cells, PAS +ve in cells
  • Dec 31, 2021, Peritoneal Fluid cytopathology: Metastatic carcinoma, favoring gastrointestinal origin. CAM5.2 + ve, CDX2 +ve , WT1 & Calretinin highlights reactive mesothelial cells.Serum CA 19-9 782 HIGH | Serum CEA 0.991 normal. Stage IV metastatic carcinoma with Primary from gastric.
  • Jan 1, 2022: Palliative chemotherapy started, considering metastatic GI CA. Half dose of chemotherapy regimen of Oxaliplatin, Irinotecan and 5 FU was given.
  • Jan 7, 2022: 20mm * 60mm uncovered metallic duodenal stent was placed in the pylorus.
  • Jan 20, 2022: Second chemotherapy was given.
  • Feb 2, 2022: A PET scan was done, which showed no FDG avid viable disease or discrete mass lesion in the gut loops or elsewhere in the body.
  • Feb 9, 2022: The third dose of chemotherapy was given.
  • Feb 15, 2022: Due to obstructive symptoms, the patient was then shifted on TPN through a PICC line.
  • March 1, 2022: NJ was placed for enteral feeding, and duodenal biopsies were taken. Duodenal biopsies were negative for any malignancy.
  • March 11, 2022: 4th chemotherapy was given. Opdivo was added.
  • March 17, 2022: 5th dose given
