For over 10 years, my sister Gia Cognata has been quietly fighting a battle that most people will never see.

What started as a medical condition requiring frequent care has escalated into something that has completely taken over her life. She now needs blood transfusions almost every week just to keep going. The constant blood loss has left her mentally and physically exhausted — her body and her mind are running on empty. The woman who used to lace up her hiking boots and take on any trail is now the one asking you to go without her, because she knows she’ll be out of breath before she even gets started. She doesn’t say it out loud much, but the embarrassment of what this illness has taken from her runs deep.

She had to stop working because her body simply couldn’t keep up anymore. I have carried her into the hospital more times than I can count. And still, she kept pushing — even as the medical debt piled up to an almost unimaginable amount.

Now, after years of trying every other option, her doctors have told her she needs a hysterectomy — a surgery she has fought to avoid for a long time, not because she didn’t want to get better, but because the financial weight of it felt impossible to bear. That choice has been taken away from her. She has no option left.

And the system is making it even harder.

The hospital is demanding roughly $8,000 upfront as a co-pay before they will even allow Gia to have her surgery. Without that payment in hand, they will turn her away — denying her the one procedure that could finally end this decade-long nightmare. Her specialty surgeon doesn’t accept Medi-Cal, meaning she will be paying out of pocket for the one doctor qualified to perform this procedure. She’s already not working, and after surgery she’ll need months to recover before she can even begin working again. Every door that should be open to her keeps getting shut.

This has become a financial trap on top of a medical crisis — and Gia deserves better than that.
