Help Ease Financial Strain for Struggling Family

This is a hard thing to do. I have been put off work for almost a year. I am experiencing memory loss and am not getting the answers needed so I can either go back to work or file for disability.

My wife works full time but it has been a struggle. If we could get this money, it will help to pay off bills and hopefully take pressure off from my family. God bless you.
