With no medical insurance, Cliff put off seeking medical attention when he started having issues swallowing. Things progressed, but he still put it off, thinking that he couldn't afford it since he was providing for his family. His wife also worked, which provided insurance for herself and their two beautiful children, but it was so expensive, they didn't add Cliff.
