Help Chloe: A Senior Dog in Need

My name is Danny and this is Chloe, well, what Chloe used to be. I got her in 2019. We then moved to a small apartment in the Bronx and stayed there until 2021. I had to move and couldn't take her with me, so I found her a home in NJ. She had more space and seemed to be in a good place. I lost contact with the family who took her in. Fast forward to today, 8.8.2025, I got a call from an animal shelter in Queens. Someone found her tied to a pole with a diaper. They said she was in bad shape, but I didn't know how bad until I got there. She has late-stage kidney disease, liver issues, and can't stand up for long. She is about 15, and I couldn't just leave her there to be put down alone, so I chose to bring her home to let her live out her life in a nice, loving place. Not one to ask for handouts, but this is my first senior dog with an illness. Any tips or anything will be helpful.
