Our Community Deserves Dignity in Emergencies
When a medical crisis strikes, every second matters. But for too many families in Oklahoma — especially LGBTQ+ partners, solo agers, and people without traditional family support — those seconds can be filled with confusion, fear, and painful rejection.
Hospitals don’t always recognize chosen family. Without the right paperwork on hand, partners can be left in the waiting room, critical decisions delayed, and patients left without the support they need most.
That’s why Yew Tree Foundation created the Emergency Medical Access Card (EMAC).

What Is an EMAC?
The EMAC is a durable, wallet-sized ID card. It carries:
• The cardholder’s chosen family contact and phone number
• A QR code linking to their legally valid Oklahoma documents (POA, HIPAA release, Advance Directive)
• A clear statement that these forms are on file and should be honored
It’s simple, discreet, and life-saving. In an emergency, the EMAC helps ensure the patient’s voice — and the people they trust — are respected.

Why We Need Your Help Now
Right now, we’re producing EMACs in small batches. But demand is growing quickly: clinics, community groups, and families across Oklahoma are asking for them.
To meet that need, we must purchase professional-grade ID card printers, blank cards, and supplies. This equipment will allow us to produce thousands of EMACs reliably, at low cost, and put them directly into the hands of those who need them most.

Who We Are
Yew Tree Foundation is a registered 501(c)(3) nonprofit in Oklahoma. We are dedicated to ensuring that every person — regardless of family structure, orientation, or circumstance — has the tools to protect their wishes and their loved ones.
All funds raised go directly to Yew Tree Foundation’s nonprofit bank account and will be used solely for the EMAC project. Contributions are tax-deductible as allowed by law.
