Father of 3 with End Stage Renal Failure (CKD)

I'm a 35-year-old father of 3 fighting End stage renal failure and Uncontrollable Hypertension. I am currently on dialysis 6 days a week for 9 hours per session. I am working to get on the transplant list to help lengthen my time left.
I've been out of work since September of 2023 and have gotten to a point where I am unsure of how I can pay bills and afford anything basic. I have been trying to get on disability since January of 2024 and still have not received an answer.
I have a lengthy list of medications I am currently on, most of which are to control the hypertension. I have limited mobility and cannot walk or stand for long periods, even with the help of a cane.
As of recent, my vehicle has broken down and had to have to motor replaced and is yet again back in the shop with a broken intake manifold. This was my means of transport to and from my 2 on average weekly appointments.
The bottom line is this, I am struggling to make ends meet. I cannot afford my diet I have to follow. I cannot afford my bills and have them pushed back as far as possible before shutoff. I want MY life back! I want my freedom back! I want to be able to see my kids grow and be there for them as they grow! I want new kidneys!
Update, as of 6/9/25 I am waiting for a hernia surgery as one has formed right next to the catheter on the right side of my stomach. Depending on how they fix it, I may have to switch to in-center treatments for dialysis for a while, which would include another surgery to install a port in my chest.
