Disabled 64 has fallen on bad times

Hi.  I am 64 years old and not in  good condition.  18 years ago I was in a car accident and had my left leg amputated (amongst other injuries}.  I have been able to make a descent living and live on  my own in a small rented house.  I had two mini-strokes over the years. Last Feb 2016 I had a heart-attack and actually died.  I was brought back by a defibulator and now have one  in my chest.  Now I can hardly see ....(I have hemoraging in both eyes. I need surgery (but am having trouble having the insurance company approving it. Since then I have had an awful time with my rent at my house and my business.  I am getting evicted from both my business and house.  I have no immediated family, except my wonderful neice and no close friends.  I don't know what to do. My only income right now is $775 month disability. If you could help a little I would e forever grateful. I need help
