We have verified Daniele Li Mandri's story with the Barrow Neurological Institute and the Italian Doctors. Daniele accompanied by his mother and medical staff will make the journey from Italy to the Barrow Neurological Institute in Arizona April 8th, 2021
The following is Daniele's story:
Daniele is a 6 year child from Palermo. He was always a cheerful, lively and carefree child ... until one day, an unforgettable day at the end of 2018, he begins to have terrible pangs in his head and begins to vomit repeatedly ... mom and dad run to the pediatric emergency room thinking of a simple virus ... instead scans and resonances remove all doubts .... Daniele has a congenital cerebral arterio-venous malformation, called AVM.
AVM is a congenital lesion of the brain and / or spinal cord; It is made up of a tangle of abnormal arteries and veins joined together without the smallest blood vessels in the brain, the capillaries. It can be found anywhere in the brain. In normal circulation, blood is pushed from the heart into the arteries, until it reaches the capillaries which are the smallest blood vessels. Embolization is a non-surgical procedure, which falls within the scope of interventional radiology. The method consists in the selective occlusion of one or more arterial or venous vessels that supply the lesion, obtained with the use of particular materials and / or substances, which are introduced through endovascular catheters.
Daniele's AVM is extremely rare as it is located on the medial margin of the left middle cerebral peduncle and has a diffuse subependymal modus along the mesencephalic stem, all occurring in the fourth ventricle, that is the cavity between the brainstem and the cerebellum.
In the brain stem there are the "breathing centers" and the centers that also modulate cardiac activity. For this reason it is an eloquent zone.
We approached Dr. Michael T. Lawton in Phoenix (Arizona) who is the president and CEO of the Barrow Neurological Institute and the chair of the department of neurosurgery. The center is certified by the American Board of Neurological Surgery.
Dr. Lawton's neurosurgical experience includes cerebrovascular disorders (aneurysms, arteriovenous malformations, cavernous malformations and strokes) and skull base tumors. The center has experience in the treatment of more than 4,000 brain aneurysms, 875 AVMs and 1,000 cavernous malformations, including more than 250 in the brainstem and other highly sensitive areas of the brain ... Barrow is the institution with the highest case history in USA and in the world for cases like Daniele's. It is universally considered as the Mecca of vascular neurosurgery worldwide, so much so that even prestigious Italian neurosurgeons have gone there and still go there to learn neurosurgical techniques. We believe that the choice of Barrow is the most suitable for Daniele in order to minimize the complication and mortality rates of a surgery that is absolutely vital at present.
Daniele in these two years has also undergone cycles of radiotherapy, which should have improved his situation by 80%, waiting to be operated on, unfortunately, however, this was not the case, radiotherapy did not give the hoped-for improvements, indeed it had no effect on him ... in January 2021 he had another heavy bleeding.
Another embolization was done urgently but it was only enough to dab to take some time ... we know that the mav is always there ... so we contacted Dr. Lawton again, who on February 16, having viewed and studied the current reports, finally allowed to proceed surgically ... the total cost of hospital, professional, pre and post operative coordination and international assistance is approx. 300,000 euros which is approx. $360,000.

Lui è Daniele, un bambino di Palermo,adesso ha 6 anni,è sempre stato un bambino allegro, vivace e spensierato ... finché un giorno, un giorno indimenticabile alla fine del 2018 ,comincia ad avere delle terribili fitte alla testa e comincia a vomitare ripetutamente ... la mamma e il papà corrono al pronto soccorso pediatrico pensando ad un semplice virus ...invece vengono fatte tac e risonanze che tolgono ogni dubbio .... ha una malformazione arterio-venosa cerebrale congenita, chiamata MAV.La MAV è una lesione congenita dell’encefalo e/o del midollo spinale; è costituita da un groviglio di arterie e vene anomale unite senza la presenza dei più piccoli vasi sanguigni del cervello, i capillari.Può essere rinvenuta in qualsiasi parte del cervello.Nella normale circolazione, il sangue viene spinto dal cuore nelle arterie, fino a raggiungere i capillari che sono i vasi sanguigni più piccoli. Quindi il sangue attraverso i capillari raggiunge le vene che diventano via via più grandi fino a ritornare al cuore. Nelle MAV non vi sono capillari , il circolo arterioso e quello venoso sono in diretta continuità.Questa condizione di aumento del flusso ematico espone ad un aumentato rischio di rottura della parete vascolare: questa è la causa di emorragia cerebrale.
Non hanno mai visto un caso del genere (il suo caso è molto raro).
Nel Febbraio 2019 fa un'embolizzazione,per provare a fermare almeno per un po' di tempo le emorragie...
L'embolizzazione è una procedura non-chirurgica,che rientra nell'ambito della radiologia interventistica. La metodica consiste nell'occlusione selettiva di uno o più vasi arteriosi o venosi che riforniscono la lesione.Si ottiene con l'uso di particolari materiali e/o sostanze, che vengono introdotte attraverso cateteri per via endovascolare.
La MAV di Daniele è estremamente rara in quanto è situata sul margine mediale del peduncolo cerebrale medio di sinistra e ha un modus diffuso subependimale lungo il tronco mesencefalico.Il tutto avviene nel quarto ventricolo, cioè quella cavità compresa tra il tronco encefalico e il cervelletto.
Nel tronco encefalico vi sono i "centri del respiro" e i centri che modulano anche l'attività cardiaca.Per questo motivo è una zona eloquente.
Ci siamo rivolti al dottor Michael T. Lawton a Phoenix (in Arizona) che è il presidente e amministratore delegato del Barrow Neurological Institute e il presidente del dipartimento di neurochirurgia. È certificato dal consiglio americano di chirurgia neurologica.
L'esperienza neurochirurgica del Dr. Lawton include disturbi cerebrovascolari (aneurismi, malformazioni artero-venose, malformazioni cavernose e ictus) e tumori della base del cranio. Ha esperienza nel trattamento di più di 4.000 aneurismi cerebrali, 875 AVM e 1.000 malformazioni cavernose, di cui più di 250 nel tronco cerebrale e in altre aree altamente delicate del cervello...il Barrow è l'istituto che ha la più alta casistica negli USA e nel mondo per casi come quello di Daniele.E' considerato universalmente come la Mecca della neurochirurgia vascolare a livello mondiale,tanto è vero che anche neurochirurgi italiani di prestigio vi si sono recati e vi si recano tutt'ora per apprendere le tecniche neurochirurgiche più innovative.Riteniamo che la scelta del Barrow sia la più indicata per Daniele al fine di ridurre al minimo i tassi di complicanze e di mortalità di un intervento chirurgico che allo stato attuale è assolutamente vitale...
Daniele in questi due anni si è sottoposto anche a dei cicli di radioterapia,che avrebbero dovuto migliorare la sua situazione dell'80%, in attesa di essere operato,purtroppo però così non è stato,la radioterapia non ha dato i miglioramenti sperati,anzi non ha avuto alcun effetto su di lui...nel gennaio 2021 ha avuto un'altra forte emorragia.E' stata fatta d'urgenza un'altra embolizzazione ma è bastata soltanto a tamponare per prendere un po' di tempo...sappiamo che la mav è sempre lì...quindi abbiamo contattato nuovamente il dottor Lawton,che il 16 febbraio,avendo visionato e studiato i referti attuali ha consentito finalmente a procedere chirurgicamente...il costo totale del coordinamento ospedaliero, professionale,pre e post operatorio e assistenza internazionale è intorno a 300 mila euro.
A questa somma bisogna aggiungere il costo del trasporto, perché dovrà partire supportato da un equipe di medici e rianimatori,in caso qualcosa andasse storto durante il percorso.
La spesa del trasporto è ancora da quantificare...la scriverò quando la sapremo con esattezza.
Daniele dovrà fare questo lungo viaggio al più presto, perché ogni giorno rischia la vita, purtroppo la famiglia non può permettersi queste spese ...
