Help needed with medical intake and medications

Hi friends and family, as probably no one knows, my eight-year-old daughter Alalynn and I are in a safe house for domestic violence. I drive my daughter every day 45 minutes to school each way so she can continue to go to the same school that she is thriving in. She was student of the month and also won the marking period for putting her best effort forward.

With moving and having to switch my health insurance, I am no longer covered by the doctor that I was seeing. I am just looking for help with the intake charge so I can continue on the medication that I have been on. One would think that doctors would normally continue, but they don't; they want to run their own tests and diagnostics and I have to pay if I want to continue care. 

I recently graduated with my CPS certificates and I'm preparing to take my state test along with my CRS ,but in order to continue, i need my health first. I am beyond extremely embarrassed and heartbroken that I'm in this situation. I'm gonna post this with the belief and hope that I can get this amount in order to make an appointment with the initial cost of intake and medication. Thank you for your time
