Help a disabled trans man relocate to safe housing

Hi, I'm Wren and almost two months ago my father told me in no uncertain terms that I wasn't welcome to rent my parent’s apartment anymore. My parents have used my poor health as an excuse to mistreat me and held my housing and safety over my head for my whole life.
The inciting incident for my being kicked out and told "maybe being homeless will grow you up" was that I respectfully told my father I didn't want to discuss the trans bathroom bills with him after he told me he supported them. I've never been allowed to set boundaries with my parents and it's now reached a breaking point. I need to get the hell away from them and start living my life and getting better.
I have had Type One Diabetes for over 20 years. I also have Postural Orthostatic Tachycardia Syndrome and Myalgic Encephalomyelitis as well as adhesive capsulitis in both my hips. My constellation of health issues presents me with a huge challenge just functioning day to day.
I hope to move in with my partner to a place where my healthcare will not be prohibitively expensive and I’ll have a carer who understands the unique challenges my disability presents.
As it is right now I’m paying over 700aud a quarter for health insurance just to have access to an insulin pump- if I move to live with my partner I can have access to everything I need for my type one diabetes management for free.
I've been in a really bad place mentally thanks to a lack of support + a pattern of manipulation and abuse from my parents. I have felt like I’m rotting and deteriorating without any care or advocacy for years now and after my father kicked me out I've had to rely on some friends of a friend to have a temporary place to stay where I feel safe and okay.
Moving overseas is going to be hard but after a lot of consideration and research- it's going to be much better for me in the long run. Because of how hard moving is even for able-bodied folks, I'm needing to spend a lot of extra money that I really wish I didn't need to. Being disabled is expensive. Things like cooking for myself and cleaning, let alone catching a 19+hour flight and packing/selling all my stuff to be with my partner are things I need a lot of extra help with. For this reason I’m hoping to fund-raise a return ticket for him to help me with the move and to have him here to look after me and make sure I'm not injuring myself while I travel. Here's a brief summary of everything I'm funding for- if I'm able to fund-raise the full 15k that would be an enormous help to me but otherwise I'm prioritising tickets and paperwork costs.
WHAT AM I FUNDING?
- Plane tickets for myself and my partner who will help me with moving ($3800aud)
- Paperwork fees ($3686aud)
- Extra luggage/shipping ($400aud)
- Meds to stock up for the first part of the moving process. ($2000aud)
- A new bed for the room I’m moving to ($1200aud)
- New wheelchair ($1000aud)
- Anything left over will cover unexpected costs are bound to come up/act as a cushion for rent and utilities
