Help Overcome Health Challenges and Financial Strain

I really don't feel comfortable having to do this, but here goes nothing.
I have reliably had side-hustles and incomes as I have been trying to get a handle on ongoing health issues. They have been going on for decades, but the severity has become -much- worse over the last five months. I do not have an opportunity to see my primary care physician any sooner than August 28th.
I wouldn't be doing this if I didn't have a plan to share. To make a long story short, while fighting these medical issues over the years, despite many difficulties, I finished my education and started my career as a teacher. I have been unable to continue this over the last few years due to the snowballing of my health problems, financial insecurity, and doors which have closed due to this series of unfortunate events.
So, what then? Well, my plan is to talk to my PCP about how things have gotten worse and try to make proper adjustments. If not, I'm leaving Boston Medical Center for good.
If I can be helped on the health front, I plan to continue my life goals, which include my career. Any job opportunities that can be used as stepping stones will be accepted along the way.
However, being flat-broke with bills -and- these issues has made me dependent on the side-hustles I referred to. These have dried up. I have no idea when they may resume, if ever. I have also sold whatever isn't "needed" here and there to stay afloat.
As far as cutting back on bills, right now there isn't much, and there are options over the next few months which I can take to cut expenses down. What is needed right now as an injection to keep the wheels greased.
I set a goal which was recommended by GoFundMe, and while it is aspirational, by all means, do NOT think that it is a hard-line goal. I quite literally mean it when I say that: even one dollar will help.
PLEASE PM me if you are on the fence and would like more information, as I want to walk the line between writing a life story here and being too vague.
Thank you to anyone who can help out at all. It is genuinely appreciated. I hope I am never in a position like this again, and if the shoe is snuggly on the other foot over the coming years, never hesitate to drop me a line and ask what I can do to help with any unforeseen emergencies!
