My mother, Delores Countee, underwent emergency surgery on August 7th 2021 - where they removed her large intestines and created a stoma for an ileostomy bag. She survived being septic, being intubated in the ICU, and an endoscopy (where they found an ulcer). Once out of the hospital after 18 days, she was moved to a rehab facility, where she improved her strength and walked, on her own, for the 1st time since the surgery.

Mom was scheduled for discharge Tuesday, September 7th....and she Could Not Wait! Through all of her struggle and all of her fighting, her one constant goal was to make it home; "I want to go home".

After 31 days of turmoil and triumph, she passed away - September 6th 2021 at 9:04pm; she was scheduled to be discharged the following morning at 11am. Just hours away from being released, she was gone, and no one can tell us how or why she didn't make it. Her death was sudden and unexpected, with no time to say goodbyes.

Please help us give mom the "Home"coming she deserves
