I have been on a medical leave of absence for over a year now.  I am in the process of returning to the practice of psychiatry.

Before I can return to work, I have to complete two three-day continuing medical education conferences and undergo a comprehensive psychiatric evaluation.

The conferences and evaluation center are out-of-state.

The fee for each continuing medical education conference is $2,500.00.  The fee for the psychiatric evaluation is $3,000.00.  This does not include travel and lodging expenses.

I will need the funds by mid-October.

The only way I can return to practice is to complete the required courses and participate in the evaluation.  If all goes as planned, I believe that I will be able to return to work in November.

Successful completion of these requisites will allow me to resume my career.  During my leave of absence, I have not been able to provide the range of services I had previously provided.  My financial resources are depleted.
