First, I want to say thank you for giving me the opportunity to be considered for assistance. 

On September 14, 2016, my wife of 19 years, Stephanie Smith suddenly passed away. She was in good health, but for whatwever reason was complaining of chest pains, when she suddenly stopped breathing. Though great efforts were made by the paramedics to revive her, she did not pull through. My wife had a small insurance policy, but not enough to cover the cost of living expenses, medical bills, and the necessary items needed to ensure that her final day of rest is memorable. I currently have the responsibilty to assist with planning her funeral, which will cause me to take time off of work. This time away from work will put me further in debt as I will not have the appropriate funds to pay for living expenses such as rent , utilities, transportation to work, and other expenses.

I can honestly say that this sudden life changing event has been overwhelming, painful, and full of sorrow, but everyday I press through as best I can, with the support of my friends, family, and my co-workers.
