He is in icu. He had a blood transfusion.
The reason it cost so much is because first it's international and secondly he's in such critical care he needs hospital staff on board with him.
He decided not to get the insurance because they couldn't afford it. So that's why it is so much.

My grandpa was on a cruise and is with great sadness he had a heart attack.
