Roots provides unique and innovative blend of primary care and complementary services to build patient self-sufficiency and community empowerment. Since its inception, Roots focused on the unprecedented needs of Oakland residents and low-income communities of color. Roots’ mission is to uplift those impacted by systemic inequities and poverty by offering culturally competent, comprehensive physical and mental healthcare, health navigation, workforce enterprises, housing outreach, employment and training, nutritional assistance, wraparound services, and advocacy. While Roots is a primary care medical home for any and all patients, Roots is particularly committed to delivering comprehensive care to those who have been most marginalized and who have never engaged meaningfully in medical care.
