History: Phoenix Transition Program is a 501{c}3 Non-profit organization in Atlanta Georgia that is Trans led and trans oriented. Open on June 15, 2020 Helping LGBTQ+ individuals on a case by case basis Our Mission Together We Rise as a community. We come together to build, to educate, to uplift and empower the LGBTQ+ community . Our Vision: Assisting TGNC individuals who are experiencing traumatic, life- changing events. As funds become available we will be assisting with food, rental assistance and more. We also focus on the needs of our black and brown community and systemic discrimination. Our organization is all inclusive of all identities and races. Services: -Care package -Traumatic life-changing event (food,utilitie, phone) -Trans Binders -The Phoenix Dream Project -Re-entry
