The Carmella Rose Health Foundation serves the underserved and often invisible populations of the city of Cleveland. We meet our clients where they live, eat and pray at homeless shelters, pantries and community drop-in centers. We identify health related and social service needs and provide intensive community navigation to those at high risk...... one client at a time!

 

We'd like to introduce you to Mike, who was homeless when he came to us for help. He was unaware of the different resources available to him. Mike was suffering from medical challenges that caused his eyesight to be severely diminished. Carmella Rose took Mike under their wing, assisting in connecting Mike with the medical community, scheduling Dr.'s consultations, transportation and getting him a phone. We are proud to be able to report that Carmella Rose helped Mike to receive a necessary eye surgery and arranged respite care after his surgery. Since Mike came to us, he's been able to seek further assistance from the community and has now secured keys to an apartment that Carmella was elated to be able to assist in furnishing. Sharing our successes on the importance of assisting our homeless community navigate the health care system and address their social determinants of health is what Carmella Rose works tireless towards.... each and every day!
