Major Depression since Sophomore in HS,

Bulging or herniated disc in his lower

back since 2008, PVNS tumors in his ankle,

Hyperacusis hearing, several medical and

orthopedic surgeries, PTSD, ADHD, and

Cholinergic Urticaria and Ulcerative Colitis.
