Medical Errors are the 3rd Leading Cause of Death in the U.S.

To address this challenge, ION Medical Safety a non-profit organization is developing the World’s first learning healthcare system the PRECISE Surgical System™.  Designed with advanced technologies including the ION Life Application, the system will revolutionize surgery by improving patient outcomes, enabling learning healthcare research, developing surgical best practices, preventing medical errors, saving lives and advancing medical science. 


Parents with infants or school age children recognize the importance and responsibility in making sure their child is provided with adequate medical care.  In regards to surgery, the risks associated with any procedure can be frightening for the parent.  
  
Many variables can contribute to things going wrong, but surgical errors are often preventable.  Surgical errors can lead to severe and sometimes permanent conditions.  These can change the course of your child's life forever and cause emotional damage for your entire family.  
  
Surgical errors that can result in harm to a child include: 
  
Initiating an unnecessary surgery 
Failing to perform a necessary surgery 
Delaying a surgical procedure 
Failure to obtain informed consent 
Failing to conduct a thorough patient history 
Anesthesia errors 
Medication errors 
Using an unsanitized operating room or instruments 
Failing to monitor the patient appropriately 
Performing a surgery on an incorrect body part 
Leaving a foreign object inside a patient 



Medical errors occurring after a pediatric surgery include failure to recognize or treat internal bleeding, infection, premature extubation and discharge.  
  
Common pediatric surgical errors occur in surgeries required to correct congenital birth defects impacting the heart, lungs, kidney and liver.  Children suffering from Appendicitis often experience surgical errors from an Appendectomy.  Failure to perform an Appendectomy surgery is also common.  Childhood cancer is another example of pediatric surgical medical errors.  Surgical intervention is often required for childhood cancer however errors during these procedures can cause more harm than good.  
  
Complications of surgical errors in children include: 
  
Physical Disfigurement, Paralysis, Nerve Damage, Brain Injury, Spinal Cord Injury, Organ Damage, Heart Failure, Stroke, Cognitive Impairments, Wrongful Death    
      
These surgical errors can lead to irreversible damage to a child.  With so many variables to manage in pediatric surgeries it is clear how misdiagnosis, delayed diagnosis and failure to diagnose a child's medical condition exists.  The risks in performing surgery are just too great to not have a system of prevention, transparency and accountability.  For pediatric surgery, the PRECISE Surgical System™ will mitigate these risks.
