In the US, we are lucky to have health insurance for routine and emergency situations.  Even then, it can be quite expensive. In other nations, there are no payment plan opportunities and money has to be given upfront for surgeries, even if they are life threatening. A person's life hangs in the balance until the monetary balance is paid in full. Atif Bhai is in such a situation. He has a family to support and four daughters who desperately need their father.
