Please help, I need spinal surgery

I am sharing my mother's story...
First let me say thank you for taking a moment to read this. About me, my condition, and why I am asking for your help.
My name is Judith Bates, and I have been living with pain for many years. I currently work as a counselor and have been doing it for quite some time.
In 2002, I was attacked from behind by a mentally ill client, thrown to the ground, and my back was stomped on by this person. At the time I was injured, but thought I was young and would heal. I thought I would be fine.
Unfortunately, this has not been the case. As I have gotten older, the injuries that were inflicted on me that day, have continued to progress and get worse.
I live in constant pain. I have to wear a back brace, leg brace, use a cane, and take medication to make the pain bearable enough to continue to work. I do not want to go on disability. I love the work that I do as a counselor. I feel I make a difference in the lives of my clients.
I have been told that in order to get relief that i seek, I need to have the following four procedures:
Foraminotomy- this relieves the pressure on the nerve root as it exits the spinal cord.
Laminotomy- this relieves the pressure on the spinal cord
Discectomy- Removes the portion of a herniated or bulging disc
Facet thermal ablation- This is done in conjunction with the decompression
I really do not understand all of this, but I have been searching for relief. I have insurance but even with health insurance I am required to pay some upfront costs that I do not have. This totals $5107.00.
These procedures are not done locally, so I will have to travel. I have someone that is willing to drive me there, but again I need to cover gas, food, and lodging. I will have to make two separate trips to get these completed. I believe I have enough sick time so I am not asking for any help with wages, only expenses related to the surgery and travel. Realistically I need about $7000.00 total.
I have never asked for this type of help before. I just don't have any other resources I can draw from. I live alone and have no other income.
By the grace of God, my church brought on a collection, and was able to donate approximately $1000 towards my surgery. This will allow me to pay the initial co-pay and set up the appointments.
I have been praying for relief, and when I found the Laser Spine Institute I feel like there is hope to be able to live without this constant pain. So this is why I am asking for your help. Again, thank you for reading and please consider helping.
