Humbly Grateful for your help

Hello, my name is James, but you can call me J.R. and I'm asking for your help, and I would be forever grateful and humbled because it takes a lot of swallowing my pride to ask for help...but this is one time I really need it.
Back in 2015 I had a lump come up that looked like a lymph node had swollen during an infection that was thought to be side effect to a medication that I was on, so the prescribing doctor took me off it. In 2016 I started having really bad ear pains and numerous trips to the emergency room and my doctor from the pain, I was being diagnosed as having a ear infection every time you turned around...then hearing became muffled and full in right ear.
In 2017, I noticed that my neck was starting to become very painful when I turned my head to the right, and started having extreme jaw pain when I would open my mouth all the way and chew, and I would become choked a lot too when swallowing because it came to pass that I have to hold me head down to help relieve the pain. In late 2017, the mass had grown to the back of my ear and causing constant fluid build up, and with that is pain, and loss of balance.
Here I am in 2018, and can not turn my neck to the left or to the right because of the pain, I can no longer look up or down unless I'm doing it with my eyes. I can no longer bend or reach up because it puts pressure on the mass and it becomes even more painful. I can no longer do physical work and this is why I'm asking for your help. I have been battling with Social Security Disability to the point where my work credits have now run out and now having to try for SSI...and Social Security still have not mailed me the papers to fill out. I can not get food stamps in my state, Tennessee until I am approved on SSI because Tennessee is now again an able bodied state...under those guidelines, I'm not eligible until approved as disabled or receive SSI and there is still a 4-6 month wait. I don't even have the money to see the ENT I was referred to by one of the trips to the emergency room to get biopsy results back...in which I have to go back on August the 6th of 2018 to have more fluid drawn off the mass because the pathologist said there was something in the cells and wants more fluid to investigate it further.
