Get Don “Coach Mac” to Arizona

Hi everyone thank you for taking the time to read our story! My dad first suffered a stroke in 2015, he is a miracle and has come 10x further than anyone could have imagined! After the first 2,5 years things took a turn and due to the unethical treatment of my dad "Coach MAC" at the care facility he was at for the last 3 years, he is now currently being treated/cared for at the hospital. He is in limbo... under the circumstances we can not find another facility in our area and the hospital is trying to release him to a care facility that is somewhere in LA with no family near and just too far. The best option for him is to be near family, as you all know we are planning a move to Arizona as a family! We were not planning on moving him before us, however with the events that have recently occurred it has become urgent we get him to AZ. We have found a wonderful, safe place for my dad in Arizona near family and need help funding his transportation and first couple months of private pay until the medical insurance in Arizona is processed, the patient needs to be present to process the application  (which is way different than California). If it were as easy as putting him in a car and continuing on medi-cal it would not be a problem, but he will have to go by medical transport and pay for his facility privately.  Any help would be greatly appreciated as we need to move him as soon as possible.
