Help my service dog Jack

Jack is my psychiatric service dog. He's a sweet, loyal, excitable 4 year old dachshund. We've been in-training together for a long time and he helps mitigate my chronic mental disabilities  in so many ways, as I'm a sophomore college student. Recently having been diagnosed with a debilitating physical disability, he's helped so much in being my rock and always there for me. He fetches my medication, alerts others if I fall or blackout, provides deep pressure grounding, and is the coolest little guy ever.
Jack has a herniated disc in his back that's causing him great pain whenever he's picked up, or playing at the dog park. If left untreated, it could paralyze him.
I love my dog more than anything and I'm dependent on him to get through my day. I want him to have the surgery to help his back as soon as possible; the slipped disc surgery has a 95% success rate. He could go back to being a normal dog again and I wouldn't have this weighing on my shoulders anymore. Please consider helping us out. Much love.
