Help with shoulder surgery

In early July, I tripped and fell in a parking lot. I landed on my left shoulder and it put me in a lot of pain. At the time, I thought it was perhaps a sprain, and if I took it easy, things would heal naturally. Two months later, it hadn't so decided to talk to my physician. After X-Rays and an MRI, it was determined that I had a full thickness tear in my Rotator Cuff of my left shoulder. 

We went to an Orthopedic doctor and they stated that our best option was to get surgery to fix it. At first, this wasn't an issue, my current job as a contractor would be finishing by the end of October, and we figured I could get the surgery when my job completed, and I could recover with unemployment for the few weeks needed before getting a new position somewhere.

Unfortunately, the doctor is requiring that we pay $841 dollars up front, before they will do the surgery. While my wife and I both work full time, and she does another job part time and I do some video game streaming during my off hours to earn money, we just don't have anything extra in our budget at this time to afford this. Right now, I don't have full movement in my left arm. I can't raise my arm upwards, without using my other hand to do it, and I can't reach outwards with my left. 

I had hoped to never do something like this, asking others for help, but I just am not sure what else we can do. If you can help, thank you. Even if you can't, thank you and perhaps share this to others.
