Zoë (11) Epilepsie hulphond, Seizure assist dog

Lieve mensen die Zoë willen helpen,

 

Zoë is 11 jaar oud, sinds dat zij 6 jaar is heeft zij gegeneraliseerde epilepsie, gegeneraliseerd wil zeggen dat heel haar hersenen epileptische activiteit genereren. Dat veroorzaakt zogenaamde 'tonisch clonische' insulten. Hierbij verliest zij meestal plotseling uit het niets het bewustzijn en trekken haar spieren onwillekeurig samen. Daarbij heeft zij veel last van absences, korte momenten de hele dag door waarbij zij letterlijk even 'afwezig' is. Beide kunnen voor gevaarlijke situaties zorgen en ernstig letsel veroorzaken wanneer het op het verkeerde moment gebeurt. Bijvoorbeeld boven aan de trap, tijdens het zwemmen etc.

 

Een bijkomend probleem bij de insulten die Zoë heeft is dat zij direct stopt met ademhalen en haar lichaam soms ook niet spontaan weer zorgt voor een normale ademhaling nadat de aanval gestopt is met noodmedicatie. De eerste keer dat Zoë een insult had kreeg ik de politie aan de telefoon die er eigenlijk al vanuit ging dat Zoë overleden was.

 

Zoë's ziektebeloop verloopt erg grillig, maar wordt met de tijd steeds erger. In de zomervakantie zat zij op een bankje te lezen toen zij een groot insult kreeg. Daardoor viel zij met haar hoofd op de stenen en moest zij opgenomen worden in het ziekenhuis. De zoveelste keer voor Zoë. Sinds haar ontslag deze zomer is de epilepsie steeds erger geworden. Gisteren (7 september) is zij weer thuis gekomen nadat zij twee weken in het Wilhelmina kinderziekenhuis opgenomen is geweest. Daar is geprobeerd met verschillende anti-epileptische medicatie de epilepsie onder controle te krijgen. Helaas lukt het niet om haar epilepsievrij te krijgen.

 

Dit betekent voor Zoë in het dagelijks leven dat zij continu in de gaten gehouden moet worden. S'nachts met infrarood camera's die alarmeren bij verdachte bewegingen. Overdag door een volwassene die steeds op moet letten of het goed gaat. Mee met traplopen, niet alleen buiten spelen, niet alleen douchen of omkleden.

 

Iedereen zal begrijpen dat dit verschrikkelijk is voor Zoë. Los van de opnames, behandelingen, onderzoeken staat haar hele leven in het teken van de epilepsie omdat ze steeds bewaakt moet worden. Hier kan zij erg gefrustreerd en verdrietig van raken. Zelfs zo dat ze tijdens de laatste ziekenhuis opname aangaf dat zij liever niet meer beter zou willen worden.. Als ouder breekt je hart als je je kind dat hoort zeggen. En het is als ouder ook een grote belasting om je kind zo te moeten bewaken. Zoë zou op haar leeftijd juist haar zelfstandigheid moeten gaan ontwikkelen, dat gaat zo niet.

 

Maar er is een oplossing. De epilepsie hulphond. Een epilepsie hulphond kan vrijwel alle bewakingstaken overnemen en Zoë haar zelfstandigheid teruggeven en ernstig letsel voorkomen. De epilepsie hulphond kan tot 45 minuten van tevoren een aanval voorspellen. Dat kan de hond doordat voor de aanval de veranderende elektrische activiteit in de hersenen zorgt voor een geurverandering. Een verandering die mensen uiteraard niet kunnen waarnemen, maar de hond wel.

 

De hond zal Zoë dan waarschuwen met een vooraf getraind teken, bijvoorbeeld haar hand likken. Zoë kan dan alvast zichzelf in veiligheid brengen door te gaan liggen en door menselijke hulp te roepen. Bij een insult zal de hond zijn kop gebruiken om Zoë haar hoofd te beschermen. Verder kan de hulphond ook mensen waarschuwen, medicatie aangeven waardoor een dreigend insult minder groot wordt, een alarmknop indrukken en hiermee dus veel letsel voorkomen. Ook absences blijven voor de hond niet onopgemerkt. Als Zoë bijvoorbeeld een straat wil gaan oversteken, maar in een absence zit of een insult dreigt dan zal de hond haar weg blokkeren en haar pas verder laten lopen als het veilig is.

 

Zoë's arts Professor in de kinderneurologie Dr. Braun van het WKZ heeft een indicatie afgegeven voor een epilepsie hulphond voor Zoë. Deze indicatie wordt ondersteund met brieven van Zoë's huisarts, school, kinderarts en Speltherapeut.

 

Uit wetenschappelijk onderzoek blijkt dat bij mensen met epilepsie stress de belangrijkste trigger voor een insult is. De hulphond neemt veel stress weg, uit onderzoek blijkt dan ook dat mensen met epilepsie die een hulphond hebben minder vaak een insult hebben en dat de insulten ook minder zwaar zijn en minder vaak leiden tot letsel en ziekenhuisopname.

 

Er is veel stress in Zoë's jonge leven. Sinds 2014 is haar moeder ernstig ziek en sinds dit voorjaar is bekend, ook bij Zoë dat zij niet meer beter kan worden. De wetenschap dat je op deze leeftijd je moeder zal gaan verliezen is natuurlijk al ondragelijk op zich. Voor Zoë ook, maar met de verzwarende omstandigheid dat de stress en het verdriet om het dreigende verlies van haar moeder zorgt voor meer epileptische activiteit. Het steeds bij zich hebben van een vriend, de hulphond zal troostend zijn voor haar. En los van de troost dus ook epileptische activiteit kunnen verminderen en letsel voorkomen.

 

Ik heb op het moment van schrijven nog niet alle bovengenoemde brieven binnen, maar die volgen later deze week. Voor nu zal ik wel vast de brief van Zoë's huisarts hieronder weergeven, daarin wordt de situatie ook goed beschreven.

 

Wat we nodig hebben om Zoë te kunnen helpen; op dit moment alleen nog geld. De indicatie is door de kinderneuroloog gesteld, dus daarmee kan Zoë in principe op zeer korte termijn een hulphond krijgen. Echter, de verzekering betaald alleen blindengeleidehonden. Het verschilt per gemeente of er vanuit de Wet Maatschappelijke Ondersteuning financiering mogelijk is. Helaas is het verzoek bij onze gemeente direct afgewezen.

 

De hond kan gemiddeld 10 jaar zijn werk doen. De kosten voor de opleiding van de hond zijn 20.000 euro. Daarvoor wordt de hond 2 jaar lang opgeleid en jaarlijks opnieuw getest of zijn vaardigheden nog goed zijn. Daarbij komt nog de aanschaf van de hond zelf, uitgebreide medische keuringen, dierenarts kosten etc. Deze kosten zijn geschat op 10.000euro. Een totaalbedrag van 30.000euro is dus nodig om dit voor Zoë te realiseren. De eerste 25.000euro voor de training en de aanschaf en keuring van de hulphond moet direct voldaan worden. De andere 5000euro zijn bestemd om kosten te dekken tijdens het werkzame leven van de hulphond.

Dear people who want to help Zoë,

 

Zoë is 11 years old, since she was 6 years old she has generalized epilepsy, which means that her whole brain generates epileptic activity. This causes so-called 'tonic clonic' seizures. She usually loses consciousness suddenly out of nowhere and her muscles contract involuntarily. In addition, she suffers from absences, short moments throughout the day where she is literally 'absent' for a while. Both can create dangerous situations and cause serious injury if it happens at the wrong time. For example at the top of the stairs, while swimming etc.

 

An additional problem with the seizures that Zoë has is that she immediately stops breathing and sometimes her body does not spontaneously return to normal breathing after the seizure has stopped with emergency medication. The first time Zoë had a seizure I got the police on the phone who actually assumed that Zoë had died.

 

Zoë's disease is very erratic, but it gets worse over time. During the summer holidays she was reading on a bench when she had a major seizure. As a result, she fell with her head on the stones and had to be hospitalized. One of many hospitalizations for Zoe. Since her discharge this summer, the epilepsy has gotten worse. Yesterday (September 7th) she returned home after being hospitalized at the Wilhelmina children's hospital for two weeks. There attempts have been made to get the epilepsy under control with various anti-epileptic medication. Unfortunately it's not possible to get her epilepsy free.

 

This means for Zoë in daily life that she has to be monitored continuously. At night with infrared cameras that alarm for suspicious movements. During the day by an adult who must always keep an eye on whether things are going well. Climbing stairs, playing outside, taking shower or changing clothes, she can't be left alone.

 

Everyone will understand that this is terrible for Zoe. Apart from the admissions, treatments and examinations, her entire life has been dominated by epilepsy because she has to be constantly monitored. This can make her very frustrated and sad. Even so that during the last hospitalization she indicated that she would rather not get better anymore. As a parent, your heart breaks when you hear your child say that. And it is also a great burden as a parent to have to monitor your child in this way. Zoë should develop her independence at her age, that is not possible under these circumstances.

 

But there is a solution. The epilepsy service dog. An epilepsy service dog can take over almost all surveillance tasks and give Zoë back her independence and prevent serious injury. The epilepsy service dog can predict a seizure up to 45 minutes in advance. The dog can do this because the changing electrical activity in the brain causes a change in smell before the attack. A change that, of course, humans cannot perceive, but the dog can.

 

The dog will then warn Zoe with a pre-trained signal, such as licking her hand. Zoë can already protect herself then by lying down and calling for human help. In the event of a seizure, the dog will use its head to protect Zoe's head. Furthermore, the assistance dog can also warn people, hand Zoë medication that reduces an impending seizure, press an alarm button and thus prevent a lot of injury. Also absences do not go unnoticed by the dog. For example, if Zoë wants to cross a street, but is in an absence or is about to have a seizure, the dog will block her way and only let her continue when it is safe.

 

Zoë's Physician Professor of Pediatric Neurology Dr. Braun of the WKZ has issued an indication for an epilepsy service dog for Zoë. This indication is supported by letters from Zoë's GP, school, pediatrician and Play Therapist.

 

Scientific research shows that stress is the main trigger for a seizure in people with epilepsy. The service dog takes away a lot of stress, research shows that people with epilepsy who have a service dog have a seizure less often and that the seizures are also less severe and less often lead to injury and hospitalization.

 

There is a lot of stress in Zoë's young life. Her mother has been seriously ill since 2014 and since this spring it has been known, also to Zoë, that she can no longer get better. The knowledge that you will lose your mother at this age is of course unbearable in itself. For Zoë too, but with the aggravating circumstance that the stress and grief of the impending loss of her mother causes more epileptic activity. Always having a friend with you, the service dog, will be comforting to her. And apart from the comfort, it can also reduce epileptic activity and prevent injury.

 

At the time of writing I have not yet received all the above letters in, but they will follow later this week. For now I will post the letter from Zoë's GP below, it also describes the situation well.

 

What we need to help Zoë; only money at the moment. The indication has been made by the pediatric neurologist, so Zoë can in principle get an assistance dog in the very short term. However, the insurance only pays for guide dogs for the blind. It differs per municipality whether financing is possible under the Social Support Act. Unfortunately, the request was immediately rejected by our municipality.

 

The dog can do its job for an average of 10 years. The costs for training the dog are 20,000 euros. For this, the dog is trained for 2 years and tested annually whether its skills are still good. In addition, the purchase of the dog itself, extensive medical examinations, vet costs, etc. These costs are estimated at 10,000 euros. A total amount of 30,000 euros is therefore needed to realize this for Zoë. The first 25,000 euros for the training and the purchase and inspection of the assistance dog must be paid as soon as the dog is with Zoë. The other 5000 euros are intended to cover costs during the service dog's working life.
