Can anyone contribute a used, well maintained cargo van with 60k or less mileage?

Current vehicle @ almost 315,000 MI requiring monthly big ticket repairs and cannot pass inspection.
Disabled with inadequately treated persistent infections 2018, now LongCovid as well.
On Eastern Long Island, NY, vehicle no longer fit for driving to the closest doctors qualified to treat me 82 mi away.
Difficulties with fine motor control and visual problems preclude email & text messages, my preferred method of communication, but can be reached by phone. Verbal communication is painful and difficult with frequent stuttering, slurring of words after about 8:30 am eastern US time. Can communicate by phone from 5: 30am - 8:25am Eastern Time, US for calls of short duration.
Please dig deep to help service dog Sunny & me obtain a reliable vehicle. Currently unable to make doctors’ appointments and shop for healthy food.
Too sick and week for public transportation even where I not severely immunocompromised.
Single female, 62. Social Security retirement+ Disability benefits only income

Long-time reliable shop owner Bill (Hampton Brake in East Quogue with smart, capable guys who knew my car) sold the shop and his competent guys retired. Nobody but Bill would pay them what they are worth these days.
Found what I think will be a good replacement at Dynamic, here in Riverhead. Eddie was recommended by several dog park friends, and I've seen former neighbors neighbors there at Dynamic since using him these last several months.
So far, his prices and labor charges have been on the low or moderate range of shops in town. And for this last fiasco that left us stranded 20 hours with no AC or water, he had me grab the part from a nearby parts shop when the flatbed came to tow me, and squeezed it out by closing time without rushing his guys so late in the day.
Unsustainable economically to continually break down & be stranded for hours on end.
Replacing my faithful 19 y/o Chevy Blazer well enable me to resume getting to Bio-reference labs for blood work, and get back to my doctors for the face-to-face visits my insurance company requires.
Sunny & I would be most appreciative to be able to meet up with healthier friends able to take her for the more frequent and longer walks she requires so she can get off lead once in awhile.
