Need Life Saving Liver Transplant

On November 18, 2017 I got sick, went to the doctor a few days later, had a CT Scan, and was diagnosed with the shock of my life …. end stage liver disease. I had no idea I was even sick. I became jaundice, (see attached picture reflecting even my eyes turned yellow), my primary care physician referred me to a gastroenterologist. I have no strength and sleep a lot due to this disease. I was unable to work, went on Social Security Disability, and have been struggling financially to make ends meet. I have been under doctor's care and going to the hospital on a monthly basis to receive blood transfusions usually up to 5 units at a time and then an endoscopy procedure to stop the bleeding in my stomach. Recently my doctor told me the procedures are no longer working and I will have to come in for blood transfusions every couple of weeks just to have the strength to stand up.
There is no cure for this disease other than a liver transplant and my doctor is recommending the transplant stating that I am an excellent candidate for a liver transplant. I have been fighting for months to get medical insurance in order to get on a liver transplant program. I now have insurance but the monthly premiums and co-pays are high and the insurance does not cover everything.
Once I get into the liver transplant program, I will need to be traveling at least 90 miles to the hospital to receive treatments, doctor visits, testing, and continued blood transfusions until I can receive a transplant. All resulting in more costs to me. But I have no choice, if I do not receive a transplant I will die.
I need money to make monthly premium insurance payments, co-pays for doctors, hospitals, and all on a weekly basis so anything you can contribute will be greatly appreciated.
