Need car help please

Hello I'm asking for help from anyone that can. My families only reliable car ha's become unreliable and unfortunately is going to cost about half of my annual income... This vehicle is the only one we have to get my wife to her ever going list of specialist to help with her failing autonomic nervous system and to get my ASD daughter to all of her meetings appointments and therapies. We are not usually ones to ask for help but with the ever growing need for medical and therapeutic help for my family and me being the only income we don't know what else to do.
