Dear friends and family of Jeff & Stacy Martin,
Early Monday morning, their 5 year old son, Jack (one of their precious twins), succumbed to a stroke during emergency brain surgery at Primary Children's Hospital in Salt Lake City, UT. It was connected to a kidney condition he developed almost three years ago. Jack passed away on January 12, 2015. It was very unexpected as he had been living a very happy and healthy life.
Jack was such a fun little boy with a cute personality and was so full of life. He will be greatly missed by many.
This campaign was created by Jack's extended family to help with the cost of memorial services and medical expenses.
