El 19 de septiembre de 2017, un día antes de que el huracán María azotara a Puerto Rico, mi amigo y hermano José Luis, recibe la impactante noticia de un diagnóstico de cáncer. Estaba acompañado de su esposa, Sylvette y su hijo Gaby de apenas 7 años.
Todo comenzó en julio, cuando José Luis comenzó a sentir una molestia en su cadera interior derecha, mejor conocida como ingle. Acudió inmediatamente a su médico de cabecera, quien le envió a realizar un CT Scan. Posteriormente, a petición de su médico, se tuvo que realizar otro CT Scan, esta vez con contraste. Su médico le recomendó que visitara a un cirujano para que le diera su opinión al respecto, ya que los estudios sugerían que había una masa de líquido de aproximadamente 6.1 x 4.7 x 5.5cm. Al ver esto, el cirujano, sin esperar la lectura de ese segundo CT Scan, decidió removerle esa masa a través de un proceso de cirugía ambulatoria. La cirugía se llevó a cabo el 12 de septiembre y la masa fue enviada a patología. El 19 de septiembre, el cirujano le notifica a José Luis que la biopsia sugería un posible sarcoma en el tejido blando de su cadera derecha. Le refirió con urgencia a una oncóloga en Mayagüez y además, le envió a hacer un PET-CT Scan con el propósito de conocer si el cáncer se había alojado en otros órganos de su cuerpo. Pero a pesar de la diligencia de José Luis en concertar esa cita, no había mucho que adelantar. Se acercaba un huracán categoría cinco a la isla. Así que no quedó más que esperar. No fue hasta dos días después que pudo salir a tratar de realizarse el PET-CT Scan. Pero no fue mucho lo que pudo hacer. En la isla no había comunicación, ni electricidad, ni acceso a servicios médicos. Su esposa y él hicieron lo imposible por tratar de hacerle ese estudio en Puerto Rico, pero no había posibilidad de ello. A 12 días del Huracán María, en Puerto Rico no había el fármaco que necesitaba la máquina que hace ese estudio; en ninguno de los lugares en los que se hace a nivel isla.
El 2 de octubre tuvieron la cita con la oncóloga de Mayagüez, quien les notificó que sin el PET-CT Scan no había manera de comenzar tratamiento. Una segunda biopsia realizada al tumor especifíca que se trata de un sarcoma indiferenciado pleomórfico de alto grado (Pleomorphic Undifferentiated Sarcoma) localizado en el tejido blando de su ingle. La oncóloga les notifica que en Puerto Rico no hay oncólogos especializados en este tipo de cáncer, el cual suele ser muy agresivo y tiende a metastizar al pulmón.
Pero Dios nunca los desamparó y puso ángeles en su camino. Esos ángeles los llevaron a conocer a otros ángeles, entre estos a quien los ha llevado de la mano en cada paso de este proceso. Se trata de Zulema Arroyo-Farley, fundadora y presidenta de Artz Cure Sarcoma™ Foundation, organización sin fines de lucro fundada ante su propio diagnostico de sarcoma en el 2015, dedicada a crear conciencia de este tipo de cáncer raro y considerado huérfano. Además, se dedican a recaudar fondos para descubrir opciones de tratamiento.
Aproximadamente 15,000 nuevos pacientes son diagnosticados con Sarcoma anualmente en los Estados Unidos. La mayoría son mal diagnosticados antes de tener acceso a un especialista de sarcoma. Comparado con otros tipos de cáncer más comunes, el sarcoma no ha sido extensamente investigado debido a que los fondos para su estudio son mínimos. Esto se debe a lo raro que es. No se tiene conocimiento de como ocurre este tipo de cáncer. No se sabe si es genéticamente transmitido. Tampoco se ha descubierto un gen, pero sí se sabe que puede atacar cualquier parte del cuerpo donde haya tejido blando, músculos y los huesos. Los sarcomas de tejidos blandos son tejidos fibrosos, vasos sanguíneos o tejidos profundos en la piel.
Al escuchar su historia y considerando las condiciones de la isla luego del huracán, Zulema teniendo conocimiento de este tipo de cáncer, la urgencia de ser tratado y ser paciente en M.D. Anderson Cancer Center ubicado en Houston, Texas, les recomienda salir de Puerto Rico para recibir tratamiento. Esta institución de cáncer es el centro de excelencia mundial de sarcoma, además de ser la institución número uno en Estados Unidos para tratar todo tipo de cáncer. Mediante sus contactos y debido a la situación humanitaria en Puerto Rico, ella logra que acepten el caso de José Luis. Además, les consigue a él y a su esposa un vuelo humanitario a Houston. Por su parte, Zulema es defensora de los derechos a los pacientes y asesora a familias que se enfrentan a un diagnóstico de sarcoma.
Como si fuera poco, Zulema les gestiona vivienda a través de otra fundación en Houston con quien colabora en casos de sarcoma: A Shelter for Cancer Families (formely Amschwand Sarcoma Cancer Foundation) . Aquí conocen a otro ángel detrás de su historia, Melissa Phipps Bellinger, quien es la co-fundadora de esta fundación, dedicada entre otras cosas, a proveer vivienda a los pacientes de sarcoma y sus familias mientras reciben tratamiento en Houston.
Una vez evaluado en M.D. Anderson por la Dra. Cormier, cirujana oncóloga y eminencia de sarcoma, José Luis recibe la noticia de que una parte del tumor que tenía originalmente en la ingle tiene márgenes positivos y aparenta no haber sido removido en su totalidad por el cirujano que lo operó en PR. El resto del tumor mide aproximadamente 3 cm y está alojado en un área bien crítica para remover. El 80% de los casos de sarcoma son mal diagnosticados por médicos que no tienen especialidad en oncología y que piensan que es un tumor benigno cuando no lo es. Por esto, es importante recalcar que no le permitan a ningún médico operarles de una masa sin hacerle una biopsia, ya que pueden poner en riesgo la prognosis en caso de que sea sarcoma.
La recomendación del equipo médico de M.D. Anderson es proveer un tratamiento de quimioterapia para aniquilar las células de cáncer microscópicas en la sangre, prevenir recurrencia y metástasis. Seguido por la radiación, con el propósito de disminuir el tamaño del tumor para poder ser operado y obtener márgenes negativos. Gracias a Dios, luego de someterse a estudio del pulmón, el sarcoma de Jose Luis no se ha metastizado al pulmón.
José Luis es fiel creyente de nuestro Señor Jesucristo. Por eso solicita, antes que todo, de sus oraciones a nuestro Padre, para ayudarle a vencer este mal. Cada momento que recordemos a José Luis y a su familia, es momento de elevar una plegaria al Todopoderoso por su salud. Sabemos que quien único tiene el poder de quitar este mal de su cuerpo es Dios. El costo del tratamiento que requiere José Luis está muy por encima de su capacidad económica. El plan médico que tiene no le cubre en Estados Unidos. On September 19, 2017, one day before Hurricane Maria lashed Puerto Rico, my dear friend José Luis, receives the shocking news of a cancer diagnosis. He was accompanied by his wife, Sylvette, and their 7-year-old son Gaby.
It all started in July, when José Luis began to feel a discomfort in his right inner hip, better known as groin. He immediately went to his family doctor, who sent him to perform a CT Scan. Subsequently, at the request of his doctor, he had to perform another CT Scan, this time with contrast. His doctor recommended that he visit a surgeon to give his opinion about it, since the studies suggested that there was a liquid mass of approximately 6.1 x 4.7 x 5.5cm. When seeing this, the surgeon, without waiting for the reading of that second CT Scan, decided to remove that mass to him through a process of ambulatory surgery. The surgery was carried out on September 12 and the mass was sent to pathology. On September 19, the surgeon notifies José Luis that the biopsy suggested a possible sarcoma in the soft tissue of his right hip. He referred urgently to an oncologist in Mayagüez and also sent him to do a PET-CT Scan in order to know if the cancer had lodged in other organs of his body. But despite the diligence of José Luis in making that appointment, there was not much to advance. A category five hurricane was approaching the island. So there was nothing left but to wait. It was not until two days later that he was able to go out and try to perform the PET-CT Scan. But it was not much he could do. On the island there was no communication, no electricity, no access to medical services. His wife and he did the impossible to try to make him the study in Puerto Rico, but there was no possibility of it. After 12 days of Hurricane Maria, in Puerto Rico there was no drug needed by the machine that makes that study; in any of the places where it is done at the island.
On October 2 they had the appointment with the Mayagüez oncologist, who notified them that without the PET-CT Scan there was no way to start treatment. A second biopsy performed on the tumor specifies that it is a high-grade pleomorphic undifferentiated sarcoma located in the soft tissue of his groin. The oncologist notifies them that in Puerto Rico there are no oncologists specialized in this type of cancer, which is usually very aggressive and tends to metastasize to the lung.
But God never forsaken them and put angels in their way. Those angels took them to meet other angels, among whom he has taken them by the hand in each step of this process. This is Zulema Arroyo-Farley, founder and president of the Artz Cure Sarcoma ™ Foundation, a non-profit organization founded on its own diagnosis of sarcoma in 2015, dedicated to raising awareness of this rare type of cancer and considered an orphan. In addition, they are dedicated to raising funds to discover treatment options.
Approximately 15,000 new patients are diagnosed with Sarcoma annually in the United States. Most are misdiagnosed before having access to a sarcoma specialist. Compared with other more common types of cancer, sarcoma has not been extensively investigated because the funds for its study are minimal. This is because of how rare it is. There is no knowledge of how this type of cancer occurs. It is not known if it is genetically transmitted. A gene has not been discovered either, but it is known that it can attack any part of the body where there is soft tissue, muscles and bones. Soft tissue sarcomas are fibrous tissues, blood vessels or deep tissue in the skin.
When listening to her story and considering the conditions of the island after the hurricane, Zulema having knowledge of this type of cancer, the urge to be treated and be patient in M.D. Anderson Cancer Center located in Houston, Texas, recommends them leave Puerto Rico to receive treatment. This cancer institution is the world center of excellence for sarcoma, as well as being the number one institution in the United States to treat all types of cancer. Through her contacts and due to the humanitarian situation in Puerto Rico, she manages to accept the case of José Luis. In addition, she gets him and his wife a humanitarian flight to Houston. For her part, Zulema is a defender of patient rights and advises families facing a diagnosis of sarcoma.
Zulema also manages housing through another foundation in Houston with whom she collaborates in cases of sarcoma: A Shelter for Cancer Families (formely Amschwand Sarcoma Cancer Foundation). Here they meet another angel behind their story, Melissa Phipps Bellinger, who is the co-founder of this foundation, dedicated among other things, to providing housing for sarcoma patients and their families while receiving treatment in Houston.
Once evaluated in M.D. Anderson by Dr. Cormier, oncology surgeon and eminence of sarcoma, José Luis receives the news that a part of the tumor that originally had in the groin has positive margins and appears not to have been removed in its entirety by the surgeon who operated on it. PR. The rest of the tumor measures approximately 3 cm and is hosted in a very critical area to remove. 80% of cases of sarcoma are misdiagnosed by doctors who have no specialty in oncology and who think it is a benign tumor when it is not. For this reason, it is important to emphasize: do not allow any doctor to operate you from a mass without performing a biopsy, since they can put at risk the prognosis in case it is a sarcoma.
The recommendation of the medical team of M.D. Anderson is providing a chemotherapy treatment to kill microscopic cancer cells in the blood, prevent recurrence and metastasis. Followed by radiation, with the purpose of decreasing the size of the tumor to be able to be operated and obtain negative margins. Thank God, after undergoing a lung study, Jose Luis's sarcoma has not metastasized to the lung.
José Luis is a faithful believer of our Lord Jesus Christ. That is why he asks, first of all, for his prayers to our Father, to help him overcome this illness. Every moment that we remember José Luis and his family, it is time to raise a prayer to the Almighty for his health. We know that the one who has the power to remove this illness from his body is God. The team of doctors who will attend Jose will be guided by Him at all times.
The cost of the treatment José Luis requires is far above his economic capacity. The medical plan they have does not cover in the United States. In addition, a supplemental plan to cover the costs of a cancer treatment is really well below its actual cost. That is why José Luis and his family need our help.
