Home Farm Family Medicine

Hi!  Home Farm Family Medicine is a new primary care practice in downtown Brattleboro, Vermont.  While we have a special focus on marginalized populations, including those identifying as LGBTQ+, people struggling with addiction, our homeless neighbors, and our home-bound neighbors,  mostly we are looking to provide good old-fashioned family medicine.  There are a lot of ways that we plan to make this work well, but cash will be tight for the first year.  If you would like to help improvements along more quickly, this is a place where you can lend even small amounts of money to the practice so that we can implement changes before we start getting paid for them.  We have also been pretty hard-hit with the COVID-19 pandemic, but we are all healthy and doing fine.

We will get by on this timeline without extra funding, breaking even in terms of cash-flow in May 2021.  With your financial help, we can move forward more quickly to equip the office and bring nursing and therapeutic staff on board.  You are welcome to donate outright, but a loan in the spirit of member economic participation is what I am hoping for:  if you include your name with your contribution, I will return your money within 12 months.  If you would like to consider a larger investment with interest, please contact me at jeremy@homefarmfamily.com to work out terms.

Here is our timeline  of improvements without any funding help:

January 2020:
     - Opened on Thursdays for high-needs patients in Brattleboro
     - Answering service took phone calls
     - Most equipment was already in practice
     - Jeremy and Hannah had a baby!  Aurora (Rory) born 1/6/20

February 2020:
     - Opened full-time
     - Early morning and evening appointments were available
     - State funded Blueprint for Health Addiction Spoke team (Nerissa and Joann) started in Brattleboro
     - Systems were established for short wait times for appointments
     - All phone calls during business hours were answered by a human in the office, rather than the answering service
     *Staffing this month: Physician, one medical assistant, office manager, two Spoke staff

March 2020:
     - Part-time medical assistant student (Nate) joined the team
   *Staffing this month: Physician, one medical assistant, one part-time medical assistant student, office manager, two Spoke staff

April 2020:
     - Full time nurse joined the team
   *Staffing this month: Physician, full-time nurse, one medical assistant, one part-time medical assistant student, office manager, two Spoke staff


May 2020:
     - Physician Assistant (Morgan) joins the practice
     - Morgan and Jeremy will share responsibility for all the patients (a unified care team for all patients)
   *Staffing this month: Physician, physician assistant, full-time nurse, one medical assistant, one part-time medical assistant student, office manager, two Spoke staff

June 2020:
     - Add full-time RN
     - EKG machine
     *Staffing this month: Physician, PA, full-time RN, two medical assistants, and office manager

September 2020:
     - Add full-time Social Worker
     - Extend hours
     - Oxygen in office
     - Hemoglobin A1c machine
     *Staffing this month: Physician, PA, full-time RN, full-time Social Worker, two medical assistants, and office manager

December 2020:
     - Add full-time Therapist/Counselor
     *Staffing this month: Physician, PA, full-time RN, full-time Therapist/Counselor, full-time Social Worker, two medical assistants, and office manager


The team so far:

Jeremy Morrison, DO - I am a board certified family physician who has been practicing in Windham County for  a little over three years now.  My special interests are child and adolescent medicine within the context of whole families, addiction medicine, transgender care, and aging in place.  

Moussou - Moussou is a Certified Medical Assistant who is helping both with visits and with building and curating the records system.  She and Jeremy have worked together previously in Putney, Vermont.  She also has a degree in Accounting from her birth country of Mali.  She has a special talent for making people feel welcome and part of our family.

Rebecca - Rebecca is managing the office as it develops.  She has many years of customer service experience and is currently working on another degree.  Our vision is to create a practice where people can get in touch easily and be seen or helped in the time they need or want to be. 

Nate - Nate is currently working toward his CMA at CCV.  He has an abundance of kindness and generosity, and it is great to see him working with people in this capacity.

Nerissa - Nerissa received her Master’s Degree in Clinical Mental Health Counseling with a concentration in Substance Abuse and Addiction from Antioch University New England in 2014. She is licensed in the state of Vermont as a Licensed Clinical Mental Health Counselor (LCMHC) and holds certification as a Nationally Certified Counselor (NCC) and Licensed Alcohol and Drug Counselor (LADC).

Joann - Joann 's work in our practice as RN Care Coordinator for our Spoke MAT program is funded by Vermont's Blueprint for Health.  She believes that everyone should be met at their level and treated with respect, and her approach to people really embodies this. 

Jake - Jake has taken on the mantle of Patient Navigator, having worked as a personal assistant and amanuensis for several years. He also has experience in IT and manages our network at HFFM.  He gets the most joy at work from helping people navigate complicated bureaucracies successfully.

Jane - Jane is a person with a checkered past, mostly in middle and high school education, but also (nothing proven in court) as a writer, filmmaker, and director.  She is both practical and creative, a good combination for a case manager.
