Help with doctor's copays

We are on fixed income, and we both have incurred rather heavy bills this summer.  We're leveraged to the hilt on payday loans, our income doesn't cover everything, and we still have to see our doctors for life-threatening illnesses.  This is for the purpose of getting us to and from appointments, covering copays and medication costs.
