My name is Amber Cloyd and I’m reaching out in hopes that you can help me help my grandma live out the rest of her life with her family.
My Grammy Carole has lived in Oregon for 15+ years and has been in an assisted living type facility for the last 4 years as she is bedridden, incontinent, and has COPD.
We had a big scare a week ago when we were notified that she was admitted to the hospital with pneumonia. Fortunately, she improved enough to be released back to the Board and Care Home she is currently in, but she is in and out of lucidity. At the same time, we were hit with the news that her Board and Care Home is closing its doors on October 30th. This leaves us not much time to find a new home for her and with her health is rapidly deteriorating, we do not know how much longer she will be around. We would like to have her transportted to San Jose to be close to us, so we can spend time with her and support her in this juncture of her life. Also, she has only met her great grandson once and hasn’t yet had the opportunity to meet her brand new great granddaughter. We all know how much joy and motivation grandkids bring to our lives!
I am currently doing everything I can to find her a Board and Care Home in San Jose or a surrounding city. Our biggest obstacle is transportation. Because of her health status, she must be transported via non emergent medical transportation. This service is very costly and ranges anywhere from 6-10k and insurance does not cover any part of it.
