Erin's Surgery for complications

In July of last year, I had a complete hysterectomy. During that surgery, the doctor nicked my bladder. Although she repaired it, a fistula formed. Since then, I have been incontinent, needing pads and disposable underwear to make it through each day.
My doctor at the time dropped me as a client once she knew there was an issue with urinary continence. She declared, without examining me, that a fistula wasn't a possibility. There was nothing wrong that hard exercise wouldn't fix. And then she dropped me as a client claiming that she felt I was suicidal. I had to seek out a new doctor.
This new doctor heard my symptoms and examined me once, before diagnosing a fistula. We confirmed it this morning.
This has cost me upwards of $5.00 per day in disposables - which works out to be around $1,800 in medical costs to date, not counting the medical bills from doctor's visits, or the new surgery cost which is coming up.
All told, I've spent around $2,000 in medical bills and other costs. Now, this new surgery is required to repair the fistula.
The costs of this new surgery will be around $5000, plus 20% coinsurance. This is a cost which we cannot survive with. With only one full-time income coming in, we are looking at bankruptcy if I go through with this surgery - and even more lifetime cost if I don't.
We scheduled the surgery, for the 27th of this month, however, the Hospital is requiring a portion of the bill in advance.
If we could pay out the medical upfront costs over time, it wouldn't be so expensive, but unfortunately, those costs are demanded up front.
