Help bring my dad home

On January 9th, 2020 my father (known to many as Scully Hunte) suffered a massive Hemmorghaic stroke where he lost his speech and movement. He also experienced multiple seizures during that time, went into a coma, and was placed on a ventilator. At the time when he had the stroke, the pandemic was upon us.
He stayed in the hospital for three months and within that time he came out of the coma and was able to move his left hand. In April during the height of the pandemic, he went into a nursing rehab center to start receiving services. At this time, all Nursing rehab facilities in NY were no longer taking visitors due to Covid-19. Unfortunately, when he was there he was mistreated and neglected. In a matter of 32 days from his arrival, he suffered multiple unstageable bedsores and was emaciated. My family and I were not informed of this until the day when they called and said he has to be taken to the hospital. At the hospital, the physicians were not pleased with the state my dad was in. He did not receive any OT, PT, & speech services (which was in his order), they did not start the process of weaning him off the ventilator, he needed multiple blood transfusions, he needed to be placed on antibiotics for infections, surgery to debrief the bedsores, and protein needed to be added to his nutrition in order for him to build his weight back up.
Once my dad became stable he was sent to another nursing rehab facility in May of 2020. At this nursing rehab, my dad was mistreated and neglected yet again. He declined even further and his bedsores became unstageable again to the point where he had to go back to the hospital because his oxygen levels and pressure dropped. At the hospital again, the physicians stated that this current nursing rehab did not do their due diligence (they did not provide rehab services and his right hand became contracted. They were not turning him every 2 hours and did not ensure that his feeding tube was working effectively. He became emaciated again.
Again, when he was stable, he was sent to another facility at the end of August, that came highly recommended because it was an advanced short-term skilled nursing facility. While there, my family and I experience a lot of inconsistencies between the nurses, medical director, and respiratory doctor. For example, a nurse mentioned that he had kidney failure, but when I followed up, the medical director stated that that was not the case. Another time, a nurse mentioned that he tested positive for Covid but that was not the case as well. The respiratory doctor told the family that the weaning process is going excellent and that my dad was up to breathing on his own for most of the day. But the nurses were saying no he is unable to breathe on his own. We were afforded the opportunity to do zoom calls with him, and during these calls, it was discovered that my father at times would not be turned for 4 hours or more. No one would come in to check in on him or adjust his feedings. While at this rehab, he was sent to the hospital for blood transfusion in November of 2020, and it was discovered that he suffered additional bedsores in addition to what he had. Once he came stable, he was sent back to the rehab.
In January of this year (2021), the facility decided to end all rehab therapy services because they could no longer bill for it. They stated that they did not see much progress. My dad was sent to the hospital for blood transfusion, and it was discovered yet again, that the facility was not taking care of him. The physicians stated that he is extremely malnourished (lack of properly ensuring he was receiving the necessary nutrients) and they were not turning him enough and his sores that began to heal when he was in the hospital from May to August became unstageable again.
Three times a charm, and at this time, my family and I are not approving for my father to be discharged back into another rehab facility. We have given it three times, and each time they have failed my father. The hospitals are saying that the rehab centers are not taking care of him, so we are not taking the risk again to let my dad go through this suffering again. Whenever he is at the hospital and we are able to visit (visits are allowed at hospitals in NY), he is more alert, he moves his mouth trying to talk, and he moves his legs. At the rehab center during this pandemic, my dad is mistreated, neglected, and alone. There has been a little push back from the hospital stating that he needs to go back to rehab and wants to discharge, but we are fighting for my dad to come home as soon as possible, where he will properly cared for physically and emotionally, and start to make progress again.
The process has been started to get him home and the following has been done:
Extensive research on my end had been done on the steps needed for home mechanical ventilation, home environment accommodation, etc.
Calling Medicare and Medicaid to inquire about home care services especially for someone on a ventilator that needs to be weaned off.
HRA and Visiting Nurse State have been called as well to know of the next steps involved to get him home.
At home my father will need the following:
1. home mechanical ventilator and a respiratory therapist/doctor to come in and continue the weaning process.
2. Diapers, pads, commodes, gauze, wound dressing, wipes, gloves, etc
3. OT, PT, Speech
4. Feeding as well as a nutritionist to come in and monitor his feeding until the trache is removed
5. Wound care that includes a would care specialist coming in
6. Hospital bed or appropriate air mattress that aids in turning and eliminating pressure on his skin to avoid skin breakdown.
Medical Care, support, and supplies are very expensive in NY and not everything will be covered by my dad's insurance, unfortunately. In 2019, my family went through many unfortunate and unexpected events that resulted in money depletion. My mom is no longer working due to the emotional toll the stroke has brought on her. There are not enough finances between my siblings and I to take care of all that my father needs. The funds received will go to all that my dad needs (listed above) that are not covered by the insurance. Some of the funds will also go into getting the best-suited home environment for him. Funds will also go to private medical specialists, and therapists as well, that will come in to ensure that he is properly cared for and if he may need more things medically.
