Help our struggle

Hi my name is Stacey moynihan i am 28 and I have been struggling with a heart condition and have been in and out of
hospitals for almost a year and a half now struggling with heart problems I have already had one open heart procedure where they replaced one heart valve and repaired another due to that surgery my heart is now functioning below 20% which is considered severe heart failure I have also been to a nursing home on 4 different occasions for intravenous antibiotics each time was for a minimum of 6 weeks I went into the hospital last week for an outpatient procedure where they put you to sleep and put a camera down through your throat to take images of your heart and when I woke up I was told I was being admitted again for the 6th time in the hospital and that I'm going to be needing another open heart surgery 
My surgeon explained that this surgery is going to be very high risk this time due to my heart failure and after I am going to have to go to another nursing home for 6 to 8 weeks for more intravenous antibiotics due to all this I have not been able to work I have applied for disability but every time I get put into a hospital or nursing home they push it off saying that all my needs are covered in these facilities so I won't be eligible until I finally get home and then I still need to wait for the approval and unfortunately due to that I am very behind on all my bills and rent so I am reaching out for a little bit of help it would be a blessing and a huge weight lifted off my shoulders so I would be able to concentrate solely on my surgery and recovery
