Losing 4 weeks pay due to LOA

Hi, my name is Nikki. I suffer from Bi Polar, Anxiety and PTSD. I work from home in tech support. Recently I had to find a new doctor as my orignal doc was retiring. My new doctor has decided to step me off my current medication as my mania is not controlled. Its been 6 weeks and the effects have hit me hard. I'm cycling rapidly, unable to sleep and spinning. Crying fits, irritability, panic attacks. This has affected my attendance at work and they have given me the choice of taking a leave of absence, or quitting altogether. I love this job and working at home is perfect for me as I have severe social anxiety. I've chosen a 5 day leave of absence which is the shortest I could choose. Thankfully my doctor is helping me thru this but the leave of absence isn't paid and I'm losing half a paycheck. My rent is half income, I live paycheck to paycheck and this loss of income will affect me heavily. Any help that anyone can see clear to offer is so greatly appreciated.
