FHCB's Bricks for Brooklyn

Bricks for Brooklyn, is a capital campaign to complete first floor renovations of the health center at 3540 S. Hanover Street. Your contribution will allow Family Health Centers to create a beautiful brick sidewalk in front of the health center. At the same time, proceeds will be used toward the cost of completing renovations. Our Bricks for Brooklyn sidewalk is a path that guides patients, family members, and visitors to our primary health care services. Regardless of a person’s ability to pay, you can receive medical and dental care, learn how to maintain your health, and get behavioral health services as well all under one roof. Our capital campaign aims to finish construction of first floor exam rooms and behavioral health services space. Our goal is to raise $100,000 to help complete first floor renovations.
We invite you to become part of the Bricks for Brooklyn remembrance. Your support enables us to have the space needed to service vulnerable individuals and families.

Making a Difference
Family Health makes a difference in the lives of families by teaching patients to control pre- diabetes, diabetes and hypertension; treating substance abuse, preserving the teeth of our children, restoring the teeth of adults, preventing infant mortality, and so much more. As we assess the changing health care needs of the communities we serve, we know that access to dental care, medication, and a patient-centered medical home persistently challenge low income and uninsured people.
You can be seen at Family Health even if you have no insurance. If your income is 200% or below poverty, out of pocket costs are deeply discounted. We never turn anyone away because of inability to pay. Our sliding fee scale is on our website at fhcb.org.

For nearly four decades, Family Health Centers of Baltimore (FHCB) has provided affordable, high-quality primary health care in Baltimore City. Our mission is to provide quality, integrated, patient-centered health care services to the communities we serve. Through health care access for uninsured and low income individuals and families, Family Health educates patients to become actively engaged in maintaining their health, managing medical conditions, and navigating the health care system. FHCB outreach to vulnerable community residents has resulted in partnerships with hospitals, the faith community, other community based organizations and local businesses.
The Importance of Philanthropy
Today, as Family Health prepares to complete renovations of our new Brooklyn Community Health Center first floor exam rooms and behavioral health space, we rely on the continued philanthropic support of our partners and friends to help us provide:
· Direct patient services for preventive care and chronic illnesses;
· Educational outreach and equipment purchases to improve patient outcomes;
· Dental services that prevent rampant tooth decay and reduce heart disease and digestive problems; and
· Medication charity care for low income patients
