PreHx (PreHistory) Challenge

The Centers for Medicare and Medicaid Services (CMS) has defined and structured nearly 30 questions that a medical provider could ask you as part of a medical interview or History. Rather than struggle during a visit to answer some of these questions, we invite you to reflect over each and answer them all of them.
Once you complete your PreHx, you can submit it is as a written request to amend your health record per the HIPAA Privacy Rule. This empowers your provider to use your words to populate the History component of your health record. This means you have the ability to co-author what is written about you.
It will only take a medical provider 30 seconds to read your PreHx. From there, he/she will be able to ask you deeper questions and conduct a pertinent examination. Because both of you have engaged in information exchange, you will find discussion of your diagnosis and treatment to include shared decision making. This means both of you will partner to improve your health and battle disease.
As you will discover, it takes longer than a typical medical visit to complete a thorough PreHx. The purpose of your PreHx is to facilitate patient-to-provider communication, reduce medical provider administrative burden and improve health record documentation.
At the least, your PreHx will help you prepare for an upcoming medical encounter. At best, your PreHx will help you co-author your medical record. Either way, the PreHx helps you be your own patient advocate.
In preparation for an upcoming medical encounter, or an encounter for a family member or friend, consider completing a PreHx. Once completed, you will be able to edit your document in order to adapt to future medical encounters.
Published clinical research has shown individuals to feel better heard and understood when submitting a PreHx. Participants in PreHx research expressed a patient satisfaction score of 97% (JAOA, Feb 2017, PreHistory)
