Diversity Health Center (DHC) is a privately owned, not-for-profit Federally Qualified Health Center (FQHC) that has provided medical care to the residents of Liberty and Long County for the past 10 years.
Our primary mission is to provide primary, preventative and mental healthcare services to the residents of our service area and to provide and maintain the necessary facilities, equipment and personnel required to ensure affordable, accessible and comprehensive healthcare to the residents of our service area, with a particular focus on the uninsured and underserved residents. Services are provided to uninsured, underinsured and homeless patients on a sliding scale discount based on their household size and income.
DHC provides health care service to patients regardless of their ability to pay.
Donations will be used to help construct a new state of the art building creating 11 additional exam rooms, providing the capacity to provide more than 10,000 additional patient visits per year. This will help reduce the number of patient assessing the emergency room for non-urgent visits. The facility will also provide an in-house pharmacy, which will provide medication for our uninsured patients at a reduced rate.
