BCHN Health & Wellness Programs

Bronx Community Health Network (BCHN) is an innovative, caring organization that is changing lives every day.  BCHN assures access to quality, affordable medical, diagnostic and social support services for over 110,000 children and adults at 18 network health centers. BCHN collaborates with more than 60 multi-sector organizations to plan and implement clinical and community programs to manage chronic disease, overcome social, economic and education challenges, and embrace healthy living.  The organization advocates for policies and resources to reduce health disparities related to preventable chronic diseases, disability and deaths. BCHN is raising money to benefit expansion of our community health and wellness programs, including prevention and management of diabetes, hypertension, asthma, HIV/AIDS, behavioral health and opioid use, as well as insurance assistance and care coordination for supportive resources.

Special days for moms with educational talks, mommies networking and sharing stories and tips, healthy baby food prep demos, prizes, and much more!

Stress Management Classes
Stress management classes help participants to better manage chronic illness.

HIV Testing Days
BCHN partners with Project Street Beat's mobile unit to host HIV testing events throughout the Bronx.

Patient In-Home Visits
Regular visits by our home visiting team help patients stay on track with health care plans.

Fighting the Opioid Epidemic in the Bronx
Community health workers help Bronx residents save lives, distributing and teaching how to use Naloxone kits to counteract opioid overdosing.
