Second Surgery Required

My name is Jason Rexrode. Being as private a person, as I am, those who know me well, know just how hard this is, for me to do. I now see no other way.
I was severely injured, at my job, on June 10, 2016. Subsequently, requiring surgery, on July 22, 2016, completely reconstructing my bicep, labarum and rotator cuff, on my right arm. A great delay in time, I know, for an injury of this severity, awaiting Worker's Compensation to approve, or deny, the surgery/claim. I have been on Worker's Compensation, since the day after my injury. As many of you know, that compensated amount is far less than you would normally bring home and is not near enough to live on, much less, as is the injured's responsibility, to maintain (pay) their portion of the insurance benefits. Due to this, I have not been able to maintain my portion. However, I have done the very best, I could, while still falling further and further behind, on every front, to the point of risking the loss of everything, not being able to work and bring a full paycheck, with essential overtime pay.
I have now been notified that a second surgery is required, to repair further unseen damage, which should commence within the next couple of weeks, or as soon as the surgery is approved, through the worker's compensation insurance company. My employer has also notified me, that all my insurance benefits will be cancelled for non-payment, as is my responsibility, as of this Friday, September 30.
This amount, for which I am seeking, will go to cover a portion of past due bills, much needed medications, not covered by worker's comp and to keep my company insurance coverage, which is imperative to covering other medical conditions, maintained on a daily basis. Without which, I would have no way to financially take care of, myself.
