Please help me pay my medical expenses

Hey guys. I'm Jenna, most of you seeing this know me at least a little, for those of you who don't, I'm a multiple sclerosis and fibromyalgia patient still in the process of getting disability benefits.
Today I talked with my dad, whose insurance I've been covered on through his work (we figured I would just stay on it until I officially received the disability benefits I so desperately need, and then I would switch to being covered on my own plan via Medicare) despite being diagnosed with these things. He's really concerned that I might not actually be covered. The current insurance plan we have expires December 31st.
I take A LOT of medications. Medications that are gonna require, in some cases, a lot longer than I might have here (supposing the insurance problem occurs, and it very likely could, apparently, after they were supposed to be keeping me) to withdraw off of. I'm scared out of my mind about facing withdrawl symptoms from 5 different medications (gilenya, gabapentin, cymbalta, amytriptyline, and norco)ON TOP OF my daily neurological issues, especially my pain, which these meds strongly put a dent in.
And apparently if they DO
keep me? Well, my meds are going to cost substantially more money per month anyways, and I really need the help. I feel like the biggest asshole sometimes, like all I do is beg everyone for money but I am absolutely terrified and at my wits end about what I'm supposed to do.
In addition to the medication issue which is far worse, I have not been able to pay my credit card in about 5 months. I'm accruing massive amounts of late fees and interest on top of what I already owed, and now it's actually OVER my credit limit. The minimum payment for that alone is $150.
I don't know what to do. I'm scared and poor embarrassed, but there you have it. Literally anything helps. Anything at all. Thank you for reading and sharing if you are unable to donate. I appreciate it so much more than you could know.
