Help Me Pay For Life Saving Medication

Hello.
There are a lot of money related issues in my life that have led me to make this page. I'm a 22 year old queer trans boy named Fox, living in Sweden with my girlfriend.
Three years ago I had emergency transplant surgery, after I fell into a coma with acute liver failure. I was able to have a transplant in time, but I have a lot of post-transplant problems. A lot of these things can be managed with medication, and I have to take medication every day to stop my body from rejecting my liver. These medicines are very expensive. I am currently running out of the medication I need to survive (Right now I am halfing my dose to try and make them last, something I really shouldn't be doing)
Those are not the only medicines I need. I have BPD, IBS, Anxiety, and other problems. I cannot afford medication for these at all. I have to deal with these pains and problems, which is becoming increasingly difficult.
When I was in the UK, i was registered as disabled, and able to collect money from the government, because I couldn't work. But I was living in a toxic environment, and in an attempt to help my depression, I moved to Sweden where my girlfriend lives, and gained a two year visa. I cannot work here. Though I am not supposed to work, I have applied to jobs, but being an english person with health issues makes it incredibly hard to find work.
Not working, first off, means feeding the family is incredibly difficult. There is usually at least one (sometimes two) weeks out of the month where we have little or no money for food, and just have to try and survive. As someone who is supposed to have a strict diet because of my immune system issues, this is very difficult. And trying to feed three people with little money is very stressful.
Secondly, my debts, which are the biggest problem. I have debt collector companies calling up several times a day because I haven't been paying them. I have literally no way to pay. I wish I did. My debts currently stand at owing student overdrafts of £2000 to the bank itself, and £2000 to a debt collecting company. My bank is currently taking £70 off me a month for not paying back what I owe, meaning the £2000 is just getting higher. I owe my phone company £200 for not paying my bill (believe me if I could afford it I would pay it off!), and I cannot cancel my contract until I've paid, meaning I keep running up more money.
I am very sick, and these issues are only making things worse. Being unable to have the medications I seriously need, being hounded by debt companies, and not being able to eat or feed my family. This situation is not getting better. It's getting worse, and I'm getting buried under it. I can't even think about other things I could need, like male clothes, bedsheets without holes in them, a fan, a phone charger that works. Everyday objects that most people have and i've had before are just out of the question.
I have a few things set up other than this gofundme page. I have a society 6 account (Society6.com/tangofox) full of art prints, tshirts, pillows and more. I have an Etsy account (Etsy) full of digital paper, and a Fiverr account (Fiverr) with loads of gigs up, such as photo retouching, reviewing and proofreading.
Please help if you can. Because I really, really need help.
