Hip Surgery and Recovery

Hi, I'm Owen.
I'm 32 years old and live in North Carolina with my wife, Cherise and our cats.
For the past two years I've been struggling with pain and weakness on my left side; in my hip, back and down my leg.  It's come and gone, but gradually gotten worse and is increasingly limiting my mobility and ability to work. I am self employed, and do not have medical leave or paid time off available to me. 


Getting treatment has been a long, slow process. 
I started seeing my doctor for the problem in January of 2013, and we explored basic treatments; muscle relaxants, basic stretches, ice, anti-inflammatory medication and steroids. They offered some relief but were all only temporary.
When I was able to get insurance through my wife's employer in February 2014, I went back for another round of the steroid treatment and got a referal for physical therapy, which I participated in for several months. The physical therapy helped with the associated muscle pain but didn't solve the problem.
My doctor refered me to an Orthopaedic specialist. X-rays have shown that I have a deformed hipjoint that is grinding against itself, and MRIs confirmed that the cartilidge in the area has been worn down. After examination and a steroid injection, we've decided that Arthroscopic Hip Surgery is the best course of treatment. 
It is outpatient surgery done through a few small incisions, during which they will file down the bone and repair some of the damage. 
This will most likely relieve my pain and once I recover I should be able to resume normal activities. Without it, my condition will definitely continue to deteriorate. 

I am scheduled to have the surgery performed on the 16th of September.

While I thankfully have insurance that is going to cover the bulk of the surgery and followup physical therapy, between what I've already paid and the delays in my being able to work, money has been very tight and I could really use some help.

Primary Goal - $1750

Fascility Fees: $1555 (Paid!)
Cold Care Polar Therapy Unit: $150 (Paid!)
Crutches: $45 (Paid!)

Secondary Goal - $1600 ($3305 Total)

Arthroscopic Hip Surgery: $750-1120*
Physical Therapy for two months: $480 

*I have moved this to my secondary goals because I found out that none of that will be due upfront, and I will probably have 30-45 days after the surgery itself to start to address the payment as it goes through insurance.

Tertiary Goal - $850 ($4200 Total)

Time Off: $700 (this would allow me a full week off from work without falling behind on bills).
Stationary Bike: $150 (strongly recommended for long term recovery).
